This is is not the IMP project but is sure to be lead by some fantastic faculty. Call in and/or visit the link at the bottom to learn more. Gordon The Challenge In order to maintain healthy lives, people with chronic, or long-term, conditions and their families have to manage their condition from day to day. This self-management often involves understanding and following complex medical regimens, and planning and following through with changes in lifestyle, such as weight loss, stress reduction, or increasing exercise. Self-management support refers to the information, education, resources, and care offered to people with chronic conditions to help them deal with their illness. The goal of self-management support is to strengthen people’s competence and confidence to manage their condition, make informed decisions about care, and adopt healthy behaviors. The Program The Institute for Healthcare Improvement (IHI) is pleased to offer a virtual learning community beginning in March 2007 to improve the capacity of the office practice to deliver excellent self-management support to patients. This program will test robust models for self-management support in the ambulatory practice. Participating teams will engage patients and family members to support the improvement work of their practice. Areas of focus will include evidence-based approaches to self-management support and system changes that support and sustain self-management activities, including collaborative goal setting, follow-up, and proactive management.
