I have recommended that this patient be fitted with orthopedic shoes

with a high wide box anteriorly and with an expanded heel counter and a

good high-arch support. My question is concerning the high wide box

and the good high arch support. Is the high and wide both adjectives

that need a comma between them or is the high modifying the wide? Do I

need the hyphen between high and arch?
