more on fpsa test and results.

Dr. von Eschenbach, a urologist who is director of the Center for Genitourinary Cancer at the M.D. Cancer Center, Houston, says use of fPSA depends on the context and the patient being evaluated. For instance, a 50-year-old man with a normal size prostate and a PSA of 7.5 ng/mL should undergo an immediate biopsy. On the other hand, a 67-year-old man with an enlarged prostate and the same PSA reading might decide to have a fPSA test rather than a biopsy. "If the fPSA is above 20 percent, it's probably due to benign growth, and we continue to watch and wait," he says.

Catalona, MD, professor of urology at Washington University School of Medicine, St. Louis, and a world-renowned expert in prostate diseases, says he uses fPSA for men with PSA values down to 2.6, because there's a significant chance of cancer even with the lower values. "About 10 percent of the population has a PSA between 4 and 10," he says. "That doubles when you go down to 2.6."

If the fPSA result is above 25 percent, the chance of a positive biopsy for cancer is extremely low. At that point, patients, especially if they're older, might elect not to have a biopsy because any cancer present is slow-growing, he says. If fPSA is under 10 percent, "the chance of cancer is high, and it's very likely that cancer would be aggressive," Dr. Catalona says. Then a biopsy is warranted.

Sure I can try and remember to. This very valuable piece of information dosent seem to be getting around to the Family Practice, Uros, Oncas like it should. What are you doing for that pain?

This needs to be dealt with my friend. I hate to see that you are suffering.

Are you seeing an oncologist or pain specialist?

Best wishes

RE: more on fpsa test and results.

Thank you Kathy, i never thought you didnt.

My pain is getting worse and my psa has jumped again from thousand's into the 10th range in two months so im worried. I understood what you were saying but probally came across wrong. Sorry

Yes im going to an onca but he's not helping. My insurance runs out at the end of this month so i guess thats the end, im not going to bankrupt my family. I'll probally get one more psa test in and am hoping to get an injection of elagard from the uro before it runs out. thats $3800.00 usd for each injection but it lasts 6 months. My plan after that is to see if i can get by on casodex. I have Vicodin for the pain and im not sure if its the cancer or the nerves in my back. It dosent feel like a pinched nerve. I see you have had some good luck with the removal. glad to hear it. I dont think im getting all the postings because i didnt know you were even going in for anything. Other postings dont seem to make sence either sometime. I havent heard from RJ in quite awhile. I hope hes well, i know he was having some bone problems also.Mick also seems to have good fortune also. Hes quite happy and im glad for both of you. I

just thought i may have done something to upset you or the group. thanks Metcalf wrote: Sam What are you doing for that pain? This needs to be dealt with my friend. This is the difference with the UK.

We have NHS, it may not be perfect but my daughter who has worked in Private and NHS would choose NHS anyday. She says that often private has old uncalibrated equipment. Also all NHS treatment except for a few reasonable charges is free*** at the point of delivery. This is especially the case for hospital treatment and GP consultations!

*** Dentistry excepted

Just what do you do in the States if your insurance runs out. Surely there should be a way of getting proper treatment!

I really feel for you!

Hi Sam

My laptop has just recovered from massive email crash and it deleted over 6 months worth of saved emails. And yes I'm having bad back pains but I can't afford any more MRI's or CT scans.

With you about to lose your health insurance----aren't they supposed to offer you an individual policy continuation by law? Yes individual health policies are expensive, but facing medical bills without any insurance would be even tougher... I forget the official term for it but employers are required by law to offer you some form of health insurance continuation under an individual policy perhaps someone here on the group will know more about it than i.

I had another thought----VIADUR (or something like it) is Hormone Therapy implanted surgically under the shoulder and lasts for a whole year....any chance this would be of help before your insurance runs out? i realize that I'm grasping for solutions here but i care about you sam, and am trying to think of something before the month---and your insurance---actually ends.

We'll struggle through this together and try to find some fun in each day to distract us from 2 unwelcome visitors pain and fear....

Hi Sam,

THAT'S IT---COBRA----That's the word I was trying to remember---THANKS to Judy Thurman once again----

Sam, Your employer is required to offer you individual COBRA Health insurance coverage---even though it is expensive it is likely to be less expensive than the kinds of bills we PCa patients face----and this will be a once-only opportunity to obtain it or to lose it.

Thanks again to Judy for recalling the name of Cobra Insurance required of employers.

I came in late to the discussion, but if

you’re out of money and insurance and you need a cheaper option to control

late stage PC a then you should be looking at DES. It is not easy to obtain,

but not impossible and the cost per month is in tens of dollars, not thousands.

You have to be aware of the potential using this has for causing thrombosis,

but that can be dealt with by using warfarin, another drug that is down the

scale in costs.

Hi Sam,

Terry is right on about DES , however let me add that Estradiol Patches are reported to be safer from Thrombosis (blood clots) because the Estrogen is absorbed through the skin and never passes through the liver which is the case with oral DES. It is believed that oral estrogens are higher risk for blood clots for that reason and that is why the patches are safer.

But on the down side, Dr. Israel Barken reports that Estradiol Patches are somewhat less effective than Oral Estrogen. However, I'd be inclined to try the safest path first---indeed that is what I am seeking by shopping various doctors, because both solutions are about one-tenth as expensive as conventional Hormone Therapy for PCa. The "catch" is trying to find a doc who will prescribe Estrogen therapy because the docs are worried about liability if you get a blood clot. I'm hoping to find a doc who will allow me to sign a waiver or release that will remove his/her risk of repurcussions.

Best wishes to you my friend, as we plod along through the darkness of this disease and the many paths we have to choose from.
