We are redoing our policy. Pharmacy only wants to see patients that are new starts on warfarin but my practice has been to see all patients on warfarin for educational needs......Anyone? Guest guest Posted July 16, 2008 Report Share Posted July 16, 2008 We currently see only new starts and those we are consulted by MD to see. We have a new policy in the works to see all. I have another issue at our facility. We educate the pt to keep a consistent level of intake for Vit K foods however a few of our MD's tell pt NO spinach, broccoli etc. There is conflicting info from the education materials and what the MD's are telling pt.
