Guest guest
This is the info I have about PANDAS treatment - I was not told by the other docs who do this a lot what the dose of Keflex was: PANDAS RX Jaquelyn, I don’t think there is a consensus recommendation for the treatment. In fact outside of our more “holistic” circles, when I last did a literature search on this about 6 months ago, there was more information that antibiotics are not to be used, and that IvIg or Plasma exchange were to be used None the less, I have seen responses to the use of antibiotics. Unless a person has had penicillin/amoxicillin/augmentin exposure of recent, I prefer using bicillin, IM penicillin. My reasoning is there is less intestinal exposure than when using the oral antibiotics. I assume she is under 60 lbs, if so the dose of Bicillin is 60,000U. the dose doubles above 60lbs. If recently or multiple times had taken a penicillin, a cephalosporin such as duricef or keflex If penicillin/cephalosporin allergic, I would use the high dose regimen of zithromax, which is 12mg/kg qD for 5 days Hope this helps, I avoid augmentin like the plague, mostly because there have been some issue with ammonia accumulation in the bodies, let alone it is the med that messes up the bowel more than any other antibiotic I have given up to 3 doses of the bicillin. I also monitor the titers coming down. I like to wait a few weeks so I don’t catch a residual effect since the antibody response can prolong a bit --------------------------------- Subject: Re: PANDAS : I hear augmentin has been getting some very bad press lately. Do you continue giving the IM bicillin once weekly? Or as long as there are symptoms? Thanks so much! Jaquelyn PANDAS I would appreciate knowing the current recommendation for antibiotic treatment for a 5yr old for a lab-confirmed and clinicially obvious PANDAS diagnosis; it seems like I heard it changed recently, thanks! Jaquelyn Bi-weekly zithromax for maintenance is suggested by one doc.
