Through their site I can:1) Do insurance verification (either batch or individual)2) Check all rejections and resubmit very easily3) Find any EOB for any one of my patients in <1 minute4) Keep track of all my insurance payments5) Autoimport the payment data from their site into e-MDsCost for me is $128/month and an annual maintenance charge of $200. There is also an e-MDs charge for the Gateway interface of $250. So it gets pretty expensive. I have asked both Gateway and e-MDs why they add the extra charges and I get response which sounds like “continuous programming, blah, blah.” As for the HL-7 interface. My local hospital system just set one up for me. It is very cool, BUT there is an ongoing maintenance fee associated with this as well. So even if the lab pays for the initial set-up, e-MDs charges $990/year “annual maintenance” which apparently some labs will pick up and others will not. Quest would not as not enough business other than HMO. I pay for annual support on those portals though. The EMR vendor is not going to pay, this is a source of ongoing income for them. There are forms to fill out with Gateway also. I strongly recommend getting EOB’s electronically, which will also mean additional paperwork, but worth it for sure! The Gateway setup is an additional $250 per year thru e-MDs plus an annual fee (I think $200) to Gateway, plus monthly useage fees. We are EXTREMELY happy with Gateway and recently added the credit card “Patient Pay” option. You can always get someone on the phone at Gateway after not too much waiting, who can help you understand denials, etc. I typically have fewer than 2% of my claims rejected, which means faster payment. Please feel free to contact me off-list if you have any other specific questions. As far as lab interfaces, we were told by labcorp that we don’t have enough volume. And Quest said that they would pay for our interface only if we promised to send them 100% of our business. We won’t do that because we have patients with insurance that requires them to use LabCorp (we also have patients with insurance that requires them to use Quest). For now, we are thinking of doing the flowsheets manually (hiring our son to do the data entry – he wants cash to pay for gas and insurance when he gets his driver’s license) to achieve Meaningful Use. Surescripts (e-rx) was not all that expensive to set up (one time fee to e-MDs), and is working great ever since we went from the in-house server to the ASP thru Tangible Solutions. We got Quest Free, as Quest Paid the $500 fee for our interface (one time only too) to AC to activate it and nobody said a thing about numbers from day one. Upkeep for both is Quests problem for 360 and AC's for the EMR side of it.... All of this is included in our annual support fee to AC which also includes annual support and updates for the MidMark ECG, and now E-Rx is Free and included our annual support fee as well. It seems that Jon (Dr Bertman) has made it his goal to make sure he has good relations at reasonable prices for us little guys who are his bread and butter customers... I think he got it early that not just having the interface but the needed relationship and price structure was vitally important in keeping us his customers and his Product Amazing Charts viable and so it seems he has made a good and successful effort in establishing and maintaining these kinds of business relationship for both ours and his own best interests... So even though our annual fee has gone up a good amount, I do have to say it's the first increase in all the years we have used their product, they are keeping up with all this PIA gov't garbage and they really do seem to establish great relationships for us with all these other vendors that really keep other associated costs way down..... Labcorp provides an application that sits in your system tray and connects over a secure internet connection aka SSL to fetch results.My health network provider does the same thing. And all the labs I use ( about 5 or so ) use this for messaging me results. A volunteer is coming from the school district to look at it the last Saturday in March and see if it is fixable. They also do our statements.There is a current movement to form HIE's- Health Information Exchanges- basically local databanks for hospital and lab results. The perceived future is for everyone's office to have a single bidirectional interface to their local HIE which would include Quest, Labcorp, and local hospitals. Some HIEs are already in existence, and many others are already being created with ARRA money. I would recommend looking into whether or not this process has started in your area before committing time or money to any individual interface. Even if an individual interface is free to you, it can still be time intensive to set up, and may be obsolete a year or two down the road.Subject: clearinghouse and lab interface mainly for eMDs usersTo: Date: Tuesday, February 22, 2011, 5:52 AM Hello all:We dove in, started “par†with CareFirst (Blues, here) and Aetna, and now need to start using the “BILL†portion of our EMR which is eMDs. SO, first, I have to choose a clearinghouse. Those of you on eMDs (and any others who would like to chime in), are you all using Gateway or someone else? And how do you find it? Secondly, we are hoping to get the lab interfaces with Quest and LabCorp. Local labcorp rep says we need to have at least 100 patients go there per month to have them pay for the interface or it will cost me $5K! Not yet sure on Quest, but we have about 80/month there, so might be closer to achieving “free†interface. So, my ? is: do your local commercial labs have these restrictions on the interface? If so, does your eMDs vendor pay it or share cost with you? (also wondering if under these CRISP folks, who are supposed to help us achieve meaningful use, might these interfaces be somehow discounted? I think our eRX will be covered under CRISP.)
