Suzie,

My daughter has never had any eye inflammation, and she has been Systemic

JRA since age 2. Sorry I don't know. I hope it is nothing more than an

allergy.

All the best,

Hi Suzie,

I wish I could be of more help but my son Josh hasn't had any eye problems

due to the jra, aside from the 'early cataracts' that his eye doctor discovered.

He has no symptoms from that, though. I was looking briefly on drdoc's

website and found the following info listed not under the systemic jra

subset but under the pauciarticular onset subset:

1) Pauciarticular onset JCA

These patients have involvement of up to 4 joints. A number of these

may evolve
into Juvenile Spondylo Arthropathy or Psoriasis. The majority of these

present
between 1 and 3 years of age. It occurs more frequently in females

and usually
present because the child has a limp. They usually have no significant

fevers or
constitutional symptoms. Commonest joints involved, are the knee -
approximately 50%, but the ankles, elbows or small joints of the hand

can also be
involved. There is a large association with Anti-nuclear Antibodies

- between 40 - 75% of
these children, and they are frequently associated with a Chronic

eye disease -
Uveitis - especially if the child is female and under 2 years with

positive
Anti-nuclear Antibodies. Routine eye examinations therefore, become

essential
and Iris irregularity may be seen on slit lamp microscopic examination.

The eye
disease can be unilateral or bilateral - the eye disease is not

related to the pattern
of the arthritis course. With Pauciarticular Arthritis in males

- there is a stronger association with Sacroiliitis and Spondyloarthropathy

with enthosopathy. There is an association in
these children, with HLA B27 gene subtype. Iritis is also common

in this group.
Another type of this Arthritis occurs with children with small joint

involvement in
the hands and usually this occurs in girls - rather than the boys,

and these patients
may subsequently develop psoriasis.

Okay, but remember this was listed in the pauciarticular and not systemic

subset!

Then, I found out a little more about uveitis:

Uveitis is a subspecialty of Ophthalmology that deals with inflammation

in or around the eye. Many cases of uveitis are caused by infections (such

as bacteria, fungi or viruses), autoimmune diseases (such as rheumatoid

arthritis or lupus) or tumours. There are a
large group of patients in which an underlying etiology cannot be found

and these are known as idiopathic.

Because uveitis has such varied etiologies and can affect any structure

of the eye, uveitis subspecialists work closely with other ophthalmologists

in the fields of cornea, retina and glaucoma as well as other medical

specialists

including rheumatologists, oncologists, and infectious disease physicians.

The treatment of uveitis depends on its cause. Commonly used methods

include corticosteroid and nonsteroidal anti-inflammatory drops. In severe

cases, systemic steroids and other medications that alter the immune system

have to be used. Occasionally, surgery is also necessary.
~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~
Anterior uveitis
Anterior uveitis, the most common form, occurs in the iris. Most often
it is associated with autoimmune diseases such as rheumatoid arthritis
and Lupus. This disease is most common between 20 and 50 years old and
can affect one or both eyes. Also called iritis or non-granulomatous uveitis,
anterior uveitis can be treated by applying warm compresses, analgesics
or dilating eye drops to help decrease pain. Steroid drops can help decrease
the inflammation. Anterior uveitis usually lasts a few days to a few weeks
even with treatment and recurrences are common.
Well, now we've all learned a little more about uveitis :-) & nbsp; Suzie,
good luck at the eye doctor appointment. You're probably right, that it's
a symptom of some form of allergy or maybe, like you said, a nervous habit.
Try not to worry too much. And be sure to let us know what they say about
your son's exam.
Take care,
Hello Suzie
My daughter (8) has systemic JRA (and is also so-called ANA positiv). She
had
eye inflammation a few years ago which was discovered early in the progress
and treated by
cortisone eye liquid drops.
It is now of prime importance for us to check her eyes regularly to avoid
this coming back (in worst case it may cause blindness if is not detected
early - they say).
Dear Jane,
First a note about Internet manners. Capitals are a no-no. It means you are
yelling.
Your eyes are probably suffering with Sjogrens Syndrome. I also have this
nasty problem as do several others on the list. You probably need to go to a
university based opthamologist (not an optometrist) and discuss getting tiny
silicone plugs put into the tiny drain holes called punctum in the eyes.
They start with the larger punctum -- one each eye on the lower lid in the
corner. If that is not enough the following month they will plug the upper
ones too. It is a simple office procedure and the plugs stay in for years
and years! You also can use the preservative free eye drops. There are
several brands. My doctor now recommends Theratears. If you eyes do not
respond well enough to the punctal plugs then you can have special glasses
made called " moisture chambers. "
I will include a link on this note for the Sjogrens site which is excellent.
I highly recommend their newsletter too.
To Jane... and everybody else...
I had problems with my eyes and contact lenses when I first developed
psoriasis 25 years ago. (Incidentally, the psoriasis wasn't actually
diagnosed until about 5 years later.) I believe the problem was caused by
psoriatic flakes of skin getting into my eyes. I had the same symptoms that
you described.
I gave up on my contact lenses.
However, I think I know enough now, to know a possible solution. I think
using something like Eucerin Plus lotion on the skin around my eyes would
help to reduce the buildup of dry scaly skin, which would prevent it from
falling into my eyes. Eucerin Plus is an over-the-counter lotion. I used
to use a precription product (Lac-Hydrin lotion) until this over-the-counter
product became availabe and my insurance would no longer cover the
prescription product. The over-the-counter product works just as well as the
prescription product.
For me, Eucerin Plus does a very good job. And if you were to only use it
around the eyes and face it would be VERY easy to use... just rub a little in
before going to bed. And if you only used it 2 or 3 times a week, that would
probably also work,
There are about 7 or 8 other over-the-counter lotions that are designed to do
the same thing. i.e., break down the whatever-it-is that keeps the dead dry
scaly skin attached and lets it be shed before it becomes big clumps.
My doctor gave me a list of these lotions, and if you (or anybody else) is
interested I can post the list.
I don't have any information on the eye ailment from RA but another
thing you might want to consider is effects from any medications you're
taking. My rheum had me on Plaquinel and I kept complaining to him of
progressively worsening blurred vision. It got bad enough that I was
starting to wonder if I should be driving. He kept dismissing my
complaints because the standard eye check-ups they give you when you're
on Plaquenil were okay. I finally decided to dump the Plaquinel and
surprise, surprise, my blurred vision cleared up almost right away. I
also found my eyes felt better over-all, less dryness and irritation.
Just something to think about. Hope you get some answers and help.
Hi Group - FYI. (You probably already know this but here goes...) Because
I'm on Placquinel, I have my eyes checked every 6 months by a
ophthalmologist (not an optometrist). He does a color recognition test (I
hear this is the first sign of trouble from the drug), then dilates my eyes
for a thorough exam. I would suggest this for anyone on this drug. It's
bad enough having arthritis we don't need to be blind too!!!
Lea,
That is what I meant by standard Plaquinel tests. I had those done
every 6 months and was told everything was fine, but I still had blurred
vision, which went away when I stopped the Plaquinel. I also know
someone else who had this happen, so I don't think it's that uncommon.
Gwen,
That was exactly what I was told, which was a laugh because I had just
had my eyes tested and got new contacts three weeks before I started the
Plaquenil and they were fine until shortly after I started taking
Plaquenil. The other interesting thing was how quickly these new
contacts deteriorated while I was on the Plaquenil. They lasted less
than half as long as my contacts usually do. It was as if something was
pitting and dissolving them in my eyes. I'm using a new pair since
going off the Plaquenil and they aren't doing this. So, even though I
wasn't getting the recognized eye damage that can occur with Plaquenil,
it was definitely doing something to my eyes and I didn't like it.
You'll have to make your own decision but I didn't think it was helping
me either and when I went off it, the only change I had was better
vision. If it had been helping me, I guess the decision would have been
harder to make.
Hi to all
Sure hope this proves out to be my problem. I have aleady talked to a re
-this. I have
been on Plaquenil for a couple of years and have had dry, blurry , and gritty
eyes. This has
been a real problem and the eye specialist that checks for Plaquenil caused
problems kept
telling me all was well--- " it must be your contact lenses--which is " crap "
because I didn't
wear them for a year or so when my RA was so bad I couldn't use my fingers to
get them in.
Thanks for the help -all of you--
BTW I stopped Plaquenil yesterday-never did see what good it did.
Hi
I really didn't quit it cold turkey......in the last few months I cut the dose
in half ,
with no effects and then started every other day, so it's not abrupt.
I will let you all know what the results are.
Just to expand on Gwen's comments, another thing we both noticed while
on Plaquenil was that our contacts deteriorated at least twice as fast
as they had before we were taking it. It was almost like something was
etching and pitting the contacts. Since going off the Plaquenil, that
problem has also stopped for me. Makes you wonder just what this stuff
does do to your eyes.
Gwen and ,
I think the decision on whether or not to stay on Plaquenil has to take
into account whether it seems to be helping at all. I stayed on it for
7 months and never felt it helped me at all. Even my rheum had to
concede it was not helping so with that, combined with my eye problems,
I decided to quit taking it cold turkey and see what happened. To be
fair, I wasn't in great shape at the time, but I didn't get any worse
and my vision and other eye problems improved almost immediately.
Shortly after, I decided to try a different nsaid (changed from Naproxen
to Relafen), and I've gotten progressively better ever since. If the
Plaquenil had been helping, obviously it would have been a more
difficult decision to make but I felt I had nothing to lose at the time
so I took a gamble. I could be wrong but my feeling is if Gwen has been
cutting back without any negative effects, the Plaquenil probably isn't
doing anything for her either and she won't have any serious
repercussions from quitting, at least, I hope that's what happens for
her.
Mark,
Are you aware of Sjogren's syndrome? Sjogren's is a specific autoimmune
dysfunction of the glands which lubricate the eyes, and can also effect the
salivary glands, causing dry mouth. It is associated with specific
autoantibodies which can be detected with blood tests. There is also a dry
eye syndrome which doesn't have anything to do with autoimmunity just aging,
dry skin, sensitivities, etc.
Sjogens often accompanies RA and other collagen diseases, or can exist as a
" stand-alone " rheumatic disease. Ophthalmologists recommend artificial
tears, preferably without preservatives, for the daytime, and ophthalmic
ointment, a light mineral oil and petroleum mixture, for nighttime.
Preservatives can be the cause of the stinging. Artificial tears without
preservatives come in small individual applicators which you throw away
after one use. I don't know what the recommendation is for contact wearers.
I would ask your rheumatologist or see an ophthalmologist.
Jean
Has anyone with RA had problems with their eyes burning? Actually it's my
eyelids - feel kind of like they've been sandpapered at times. I put in
some moisture drops, which can sting like crazy at first (kind of bizarre I
think), and they help a bit but not much. Seems to bother me more at night
and first thing in the morning, but can on some days persist throughout the
day. Causes me to blink a lot. Perhaps is allergies, as I've been sneezing a
lot lately - maybe the dead leaves - not sure.
If you have RA, do you think a yearly appt w/ an opthomologist is necessary
or would an optometrist suffice? None of my docs have suggested I should be
doing this, but with my eyes bugging me, I'm wondering.
Mark,
I have had similar problems with my eyes. I did go to an optometrist and
found my prescription had changed dramatically in the last six months.
My eyes still have excessive itching and dryness but at least I see
better. I didn't mention my arthritis to him because at the time I
didn't make a connection or I would have asked his opinion. Can't hurt
to get them checked, especially if you wear contacts like I do. By the
way, my prescription change in the last three years prior to this had
been minimal and just in one eye.
Mark,
I have seen an opthalmologist every year since I was 9. Children
with milder forms of arthritis frequently develop iritis, an
inflammation of the iris. Luckily, I never did. Even during the
13 years my arthritis seemed to be in remission, I was told I
should keep having my eyes checked yearly. I also experience the
dry, burning sensation in my eyes, and it's worse in the morning
when I wake up. Sometimes when outside on a windy day, they can
get so irritated that they actually feel swollen.Ma For the past
year I have been experiencing throbbing pains in the eye sockets,
which worries me, but so far, so good. I'm sure that adults with
rheumatoid or other types of arthritis are at a greater risk for
eye diseases. The fact that my mom has iritis is another reason I
keep going.
I would go to an opthalmologist, not an optometrist. An
optometrist sometimes checks a person for glaucoma, but they
don't have the experience or special equipment to check for
auto-immune eye diseases, such as iritis and uveitis. An
opthalmologist puts in special drops to dilate the pupils so they
can check for any possible abnormalities or early eye disease.
Iritis and uveitis seen in those with various types of rheumatic
disease can be there without one even knowing. It definately
can't hurt to see an eye doctor once a year. Eyes cannot be
replaced!
Take care,
Kyla
I also had a dramatic change in my prescription in the past year
since my arthritis came back. I have always been very
nearsighted. In the past several months, I started experiencing
eye pain and blurring and having trouble focusing on things. So I
went to my opthalmologist and he was shocked to see how much my
prescription had changed. But it wasn't my actual eyesight that
had changed. I had developed a severe astigmatism in both eyes. I
had only had a very slight astigmatism in my eyes prior to this.
He asked if I was pregnant or on the Pill, which the latter I
was. But I have been on them for many years, and it has never
affected my eyes to such a degree. The water retention that
pregnant women and women on the Pill experience is responsible,
because it puts pressure on the eye, changing it's shape from
round to football shaped. Are you on prednisone or any other
steroid for your arthritis? I imagine the water retention
associated with steroid use could possibly cause pressure in the
eyes also. Just a thought. :)
hi mark
i am going through the same thing right now. went to the ophthalmologist
and we first put in plugs in my drain ducts to see if that would help. they
are still in, but didnt make a differance because im not generating any
tears. so now i use a natural tear product everyday to keep them from
burning all the time. i also use celluvisc when i go to bed....that really
feels great at the end of the day. its a bit thicker than drops.
sorry to hear you are going through this same thing
