ACL repair failure

I have had an extra cap repair done on one of my own dogs. She is a F/S

Labrador 75lbs. We did a standard securos ligature repair from a surgeon I have

faith in but 1 week postop we had some significant infection/cellulitis whicn we

brought under control with Baytril/cephalexin. After all the swelling decreased

the stifle had some drawer and now at 4 weeks post op there is significant

drawer. The suggestion has been made to wait until 8 weeks to assess whether we

need a redo and what type would be appropriate. Does this sound reasonable or

do we need something sooner?

Sounds like you will need a redo but I would wait the 8 weeks just because I am a weeny about re infection.

I would worry about there being something hiding in the securos so that even when you redo if you remove the crimp at the time of surgery you may end up still having infection issues.

My mother in law's ankle fusion got a pseudomonas infection and she has been on ciprofloxacin for 5 years.

I would not waste 8 weeks- infection is no greater risk now than in 8 weeks-iwe are talking managing soft tissue and staying clean-get the stifle stable now- anyone around who can do a geometric repair vs a lateral suture?

Hi Tracey,

I have been using the Securos system exclusively for 6-7 years. Very few

problems except for few failures with very large dogs or owners that didn't

believe that exercise restriction was required post op, mostly nylon breaks at

bone anchor although had one that I in accurately crimped. I recently had one

that failed due to infection- the osteomyelitis loosened the anchor and it

pulled out of the bone. Staph that was sensitive to almost everything. I opted

to pull the entire hardware and keep the dog on Cephalexin for 1 month. Then I

went back and placed Securos's version of the tightrope (XGEN-CCR). Did use the

same hole in lateral femur since it was where I needed to place the new

implants. Continued Cephalexin for added 1mo and so far, so good. Food for

thought based on my recent experience.

No we have not removed anything yet because the infection would appear to be

under control. The swelling in the soft tissues is completely dissipated and

the joint has had no effusion for 2 weeks now. The problem now is that likely

because of the infection the repair has failed. The initial plan was to let the

joint heal and then possibly take the suture out but now it would appear that a

redo of some nature will be necessary.

Short update. We removed the repair which had pulled through the fabella and we

are booked in for TTA surgery next week.
