I have a 10 yr old 60# girl, with chronic pancreatitis on a 20gm fat diet (ordered by specialty in another city) and our peds doc just ordered clear liq w/PPN x1month to rest the pancreas. The pharm and I disagreed with the ppn but the doc won't hear it so we started her on a premix (4.25% and 10% dex) no fat for now at 50cc/hr. The plan is to discuss the case on Monday when all parties are present. She was eating her foods w/minimal pain x2 1/2 days--enzymes wnl times 6 meals. We are a tiny facility without the expertise for handling this situation. On Friday I saw the order to have her tx to Loma but today rec'd the page for PPN--the doc wants a PICC line on Monday. Any suggestions on how to feed TPN (PICC) for 1 month if it goes this way? Any good references for not using TPN? Any/all input would be appreciated. TIA, Sherri Caddy, RD, CDE sac@...
