Classic is from Cigna today.Asked about preexisting conditions and asked...During the period of December 25, 2009 to March 25, 20101. Please provide all dates of treatment and diagnoses, including any medications prescribed for this patient. 2. Please provide the name(s) of any other health care provider(s) who treated the patient during this period of time.My reply...===============================Please use common sense.Patient's birthday was March 25, 2010.
