I have only seen this once....done with a sheet. The midwife stood at the foot of the bed (end lowered) the patient grabbed one end of the sheet, the midwife had the other and it looked as if the mom was being pulled while water skiing. Her support folks supported her legs. I only had walked in to set up the warmer for the baby, but they had pushed this way for about 20 minutes.

I like it and think that it generally works well for those having some problems getting the hang of pushing or those have a hard time getting the baby under the pelvic bone. I usually use a rolled up bath towel. Semi - High fowlers or whatever is comfortable with feet on the bed... or legs held my support people. Also works if you lower the foot of the bed as low as it will go so mom can be in a sit/squat position. I hold on to the ends of the towel and have her grab the middle. Make sure and brace yourself or you can end up flying! ( I usually put a knee on the bed)

We use it with Mom in a semi sitting position. Someone stands across from her and they play "tug of war" basically. I don't use it very often, only if Mom has been pushing for a little while and doesn't seem to be making much progress. Some Moms really prefer it, others don't care for it too much. I used it in a combination with side lying pushing, and helped to turn an OP babe the other week. That Mom liked it so much, she wouldn't give the sheet up. LOL Some people like to use it the squat bar instead of someone else pulling, plus Mom can put her legs up on the bar. We usually just recruit family members to be leg holders.
