PS: Gordon -- I have a "lower overhead" solo practice, have 2.5 FTE support; EMR. I'm not as "paperless" as others-- can I still qualify as a small practice "IMP"?

opportunity knocking

i had a visit today from an administrator at a mental health facility in kirkland, washington. he's previously come to visit me, and has been impressed with low overhead, patient-centered care, emr, etc, the usual unimportant stuff.

his facility occupies a particular niche, a locked facility for psychiatric d/o's, including a dual diagnosis unit for teens. it's in a posh suburb near seattle, near bellevue. gwen, are you paying attention?

by chance, i asked him if they had an fp at their facility to take care of the non-psych stuff, and he said no, they take patients down the street to the local hospital er.

very expensive, and often inappropriate, and also often not resulting in appropriate care, i observed. he agreed.

i recalled how in residency, we fp residents on call had a plan so that if a patient from the resident clinic came to the er inappropriately, or required sooner rather than later f/u from the patient's resident physician, the er would call the fp on call, who had blank spots in the next day's schedule, so the patient could have f/u right away. worked well. fewer patients admitted or sitting in the er for hours, and not getting care.

i suggested a similar thing for him; if they had a local fp who could take their calls, triage them, and offer same or next day f/u, that would relieve the er, and get patients more appropriate care faster. i told him the calls to the doctor would have to be paid for, and he also agreed. no, i didn't have him hypnotized.

i told him about gwen in bellevue, and so he should be calling her.

perhaps that's a niche market for others of us, to take call for example for a group home, or some other such facility, like assisted living. doesn't have to be a nursing home or even a geriatric facility.

i even came up with a slogan-- "appropriate care in a timely fashion".
