I left because so many of the discussions were not applicable to my practice. Example: the discussions about why some doctors could not understand why they received warning letters from UHC for coding >50% of their visits as 99215/99205. I have learned a ton about billing, coding and fighting denials from that group, and the answers are usually quick in coming and well documented. No, the MD in Drain is not on AAFP Practicemgt listserve.. Following what most MDs do in practice management is what got us into this mess in the first place. I could give you a long list of what is wrong with the system as it is, I suppose, but in general I think it is bad to continue to read things that imprint you with bad advice. If I want advice on coding, I ask coding specialists, not MDs. I avoid most of our honored management groups. Example: Medical Economics, that nasty free magazine they send out to all of us...Had an article on increasing your bottom line, with six MD "experts" who gave bottom line advice. The first article was about how awful it was to see less than 35 patients in a day. The man actually seemed to mean it. He said one should fire anyone who does less or "train" them by having them follow a really fast MD who saw 75 patients in a day. Every time I see someone with a disease that could have killed them, which was missed by their 10 minute visit MD, I think about that article. Example: my residency managers took some management courses from some well admired medical management people back in 2001. These admired people showed them how to do something called 360 degree feedback. I would tell you more about this diabolical system for evaluating your employees if you want to know (my brother is a hot-shot second man in Weirhauser, so we talk about all the upper level management stuff for dinner conversation). By the time they were well established in this system they had lost all their old employees, could not keep employees longer than 8 months to a year, and are still going strong in having poor employee morale. It still hasn't occurred to them to get rid of the management people, after all, the health care system admires them so... Until you can show me that some MD group actually changes this dying system for the better, I am not listening to them. After reading it I thought: Thanks for the earth shattering advice! See 35+ patients/day & you will make money! Of course who cares about the quality of the patient’s care or the poor doctor’s social/family life. Besides, I could not generate enough demand in my community to see 35 pts/day every day even if I wanted to, unless I took every patient that walked in the door (ie, all Medicaid, self pay, BWC, drug seekers, etc).
