triage area

We are in the planning stages for a new OB unit and would like to include a L & D triage area. We do not have that at present, we use L & D beds for obs and triage. It ties up our labor beds as I am sure you can imagine. Several of us have been put in charge of designing the triage area and were asked to think of things we would want it in...like do we want recliners or beds, etc. I would like to know what those of you with triage areas have and what you would like to have if you could redesign yours. The best triage unit I have seen was at my prior hospital. We had 4 small rooms, no bigger than an ER room. They had stretchers, a recliner for one SO and a restroom between 1-2 and 3-4. Had a small TV, bedside table and a cupboard for wall mounted VS machine and storage and a space for the monitors underneath, so we could change to a twin monitor if we had to. They were private, but not too comfortable (we had some patients who were frequent flyers and would come in for a meal and sleep, this really deterred that. We also had a small family waiting room with a restroom for the visitors that show up when Suzie is in false labor.)

Where I am now we have 2 triage rooms set up similarly, and one ob/antenatal room we can use to triage also. If it gets busy they will put pts in a labor room as a last resort, we have also had to use our PACU which has 2 stretchers next to the triage room in a pinch.

we have carts. We can also use our tirage rooms as extra recovery rooms if needed so we have recovery equipment and FM equipment. Sometimes someone will have to spend the night so cots work ok...we even do a delivery if need be when the LDRs are full...not too often. They are small rooms though.

At the hospital I most recently worked at, we had 6 triage rooms, all with fetal monitors that can accomodate twins as well, the dynomap is on a rolling stand to go from room to room but all rooms have phones, sinks and private bathrooms. we use exam tables with 3 drawers in the side where we keep chux, ky, stuff like that and then there is a cabinet over the sink with sheets, gowns, etc in it.

There is one chair for a s.o. and we limited the visitors to one per triage room they can rotate back and forth but only one at a time, there is a computer in each room for charting and the monitors are hooked up to the central monitoring computer so it works out pretty well. There is a "labor lounge" where if triage is full we'll have some pts wait but for the most part they stay in the triage rooms. There is also a large waiting area outside of l and d for additional family members to wait there is a tv out there, a pop machine and a fish tank. unfortunately it is an unsupervised area so sometimes visitors get bored and carve their names into the chairs, poison the fish,etc... but all in all it works pretty well

We have a 5 bed triage unit with beds(these stretchers are good for those patients that arrive by ambulance). We triage patients that are 20 wks and over until 6 wks pp. During the day our perinatal unit will look after NST's for decrease fetal movement that we would normally see at night. We have one nurse and one unit assistant that staffs that unit. This unit is located between our high risk and our low risk unit and can feel quite isolating at times. Our biggest problem is these beds being taken up with early labourers. You know the kind that aren't really doing anything to their cervix but are contracting........ One of the other hospitals near by have an early labour lounge that the patients hang out in and watch tv etc without taking up a bed.

Our unit has it's own stock closet complete with iv stuff, linen etc. we have our own fax machine, a filing cabinet that contains our maternal databases and prenatals.

I love triage! What a challenge to your assessment skills. Makes me miss working in ER sometimes.

one more thing. all our bedsides are equiped with O2 and suctions and we have a delivery kit on hand. we also have a "panic"button that we carry. one button activates medical assistance, the other security. each bed is also equiped with a monitor complete with bp's and procedure gloves, soakers, linen and KY. The bathroom is stocked with UA/C & S containters etc. Hope I didn't miss anything else!

--Our triage room has 4 beds in it with very little privacy, it was

not used much until the last 3 months . We are going thur a baby

boom. Our beds do not break down, we have had 50-70 delivrys in the

triage rm, some of them have gone bad, we really needed to be on the

birthing bed, so think ahead, make them mini l & d rms.

Just curious........why do they call it a triage unit when you are delivering babies there? Are they being used as an overflow? If so, are you properly staffed for that?

Our triage is strictly triage........assess and get them out. If a patient happens to be there more than 2 hours, we have to explain why. Usually it is because a labour hasn't declared itself or we are waiting for bloodwork etc. We are equiped for delivering there (suction, 02, birthing kits) but deliveries are rare.
