Guest guest
I don’t dispute the fact that interferon can cause serious problems. I already knew that. My estranged husband refused to tx years ago because of his mental instability and knew it could send him over the edge. I did a lot of research back then and that was at least 10 years ago and long before I considered my own tx. At each visit with my hepatologist during tx, I was asked to complete a questionnaire that I called the “Suicide Survey” with questions such as, “Do you feel sad?” and “Have you made excuses not to see friends or family?” and “Have you thought about suicide?” They did not take the possibility lightly. My objection is that someone starting out on tx shouldn’t be led to believe that depression is inevitable. This tx is scary enough and it’s just not true. I may not have taken a poll of 50,000 patients, but I do know a handful of close friends who lived it and none of us were on anti-depressants. Is it an easy treatment? No. We all know that. You are absolutely right……..your outlook and support structure mean a lot. I tried to see past the 24 weeks and knew that it wouldn’t last forever. I came close to quitting when my arthritis got so bad, I could hardly get out of bed. I got encouragement here from the members of this group and from my family and friends which meant a lot in keeping me going. I am not suggesting that anti-depressants don’t play an important role in the tx of HCV……..just not in every case. Guest guest
I think the anti-depressant issue is a personal circumstances situation, based on the fact that interferon has been proven to cause depression. My Dr suggested it when I was on treatment, because I already had bouts of depression regarding infertility (we never had kids). Im glad I went on Lexapro , it helped during treatment and overall. I still ended up divorced, but that was bound to happen either way, as my ex had unkind words about my hep (and many other things I dont need to share here lol). So, if you feel a tendency toward depression and are going on Interferon I think its a good idea to take them. Ok thats my two cents. > > Quick question. Pharmacopsychiatry. 2003 Dec;36 Suppl 3:203-6 ABSTRACT Interferon alpha (IFN-alpha) is a cytokine that is widely used for the treatment of chronic viral infection or malignant disorders. During treatment with IFN-alpha, severe neuropsychiatric syndromes may occur such as depression with suicidal ideation, paranoid psychoses or confusional states. The neurobiological correlates of these side effects are widely unknown. Besides induction of other cytokines and hormonal changes, IFN-alpha has been shown to modulate the opioid, serotonin, dopamine and glutamate neurotransmitter system. Positive therapeutic effects of antidepressants such as selective serotonin-reuptake-inhibitors (SSRI) or of opioid receptor antagonists support the hypothesis that neurotransmitter changes play an important role in the development of IFN-alpha associated neuropsychiatric side effects. Guest guest
That is my point. I was never arguing the fact that interferon has been known to present problems associated with depression. I just objected to the suggestion that just about everyone will have problems and should consider the use of anti-depressants. I feel very fortunate to have completed tx without the use of any more drugs beside Procrit. Anti-depressants alone can cause a multitude of problems so why add those to a tx that is already challenging…..to say the least. I hope that massgal and anyone else looking to this group for advice and answers will understand that because Jackie states that “nearly everyone” of the 50,000+ people she refers to did need anti-d’s, it doesn’t mean that there isn’t another 50,000 who didn’t. Lots of people lose their hair…….lots of people don’t. Lots of people experience insomnia and lots don’t. Let’s just say that your mileage may vary. Those just starting tx have enough stress about their illness and tx without having to worry about something that not occur. Many people DO require anti-d’s to help them manage through tx and for those people, they are a Godsend. Guest guest
There seems to be a trend of those with prior depression needing more help while on tx. I know I have been depressed all my life and am now, 1 year post tx, still fighting to find the right pill or combination of pills to keep me off the floor. Perhaps it too is an individual need that people on tx need?Sharon in NW Washington Knitting is...time and love made tangible. Guest guest
I don’t believe your genotype has anything to do with depression or the need for anti-depressants. It could, however, have some bearing due to the fact that some genotypes require longer treatment. The longer the treatment time, the longer the time when the interferon could cause problems whether they be physical side effects or mental/emotional ones. Guest guest
I dont think genotype has anything to do with depression unless it correlates with the amount of time one needs to do treatment. Guest guest
not that I know of .. genotype is more important when trying to figure out how long one has to do treatment.. but geno 1's and 4's are harder to treat with geno 1b the hardest. Geno 3 tend to get 'fatty liver' and cirrhosis quicker than some of the others,, geno 2 is the most easily treated genotype. but unfortunately, they all move toward cirrhosis if not treated and they are active. Guest guest
I thought this was a good plan too until I talked to my Doctors about needing it during treatment. She explained why I needed to start now and not wait until I had no serotonin left in my brain. It took 2 weeks of feeling sick to my stomach and adjusting the time to take it that worked best. Then it took 2 months before it started to work. If you had a problem it would be too late to fix it or it most certainly would make me even sicker while on TX That's why I chose to start now rather than wait until I hear voices or some horrible thing like suicide is attempted. Personally I have a very stable life and I still freaked out about the HepC and the antidepressants have made it so much easier to deal with all the gawd awful tests and Doctor visits and that fack that I need a liver transplant. > > > > > Quick question. Guest guest
what I think Liz was trying to say is that genotype has nothing to do with how much damage you have,, duration does,, if you're a genotype 2 and have had it for 30 years, you can expect to have damage,, same thing if you're a geno 1.. but if you've only had it maybe 5 years, you might not have much damage. Most in America have geno 1 or so they say, ppl in china typically have another genotype, but all genotypes are found in every continent throughout the world..JackieSubject: Re: Anyone not anti-depressants during treatment? To: Hepatitis_C_Central Date: Thursday, June 26, 2008, 10:19 AM "duration a person has been sick"? Guest guest
the test that shows if the virus is active is part of the biopsy.. they tell what stage of damage you have, thats rated from 0-4 with 4 being fully cirrhosed, stage 0-1 being little to no damage.. THEN they also give you a GRADE of inflammation, that is the part that tells if your virus is active,, its graded on a 0-1 to 4 as well with 4 being very very active and 0-1 being low activity. When they do the blood tests, the PCR, it tells if you are positive to the virus antibodies and your viral load will tell if you have any virus in your blood.. see some ppl test pos to the antibodies but when they do the pcr, they have no viral load which means that their own immune system put the virus into remission.. but the viral load when you have an active virus can vary alot.. so the viral load, if you have any doesnt really tell you how active the virus is, just that its alive and cooking.. so to speak.. does this make sense? if not, let me know and I'll try to explain it again.. its very confusing at first to make sense of all of this.. Guest guest
As the original owner of this group, nurse with over 26 years experience and as a person who has been through treatment twice. I have to agree with Jackie. I had to be on anti-depressants during treatment. Both times. I would not have made it. Even with the anti depressants I would have times that I thought about ending it all. It was just the medications, and the ravages of the disease. But I think anyone who does treatment it is a must to take anti depressants. It just helps with alot of the mental side effects that you get. And that is my opinion and If anyone has a problem with what I said then they can deal with hash it out with me. Guest guest
It’s interesting since I feel that I was depressed to begin with. I was unemployed, my marriage had fallen apart, I had recently been told I could lose my sight, I was struggling with other medical problems and had financial problems. I didn’t take anti-depressants before, during or after tx and I feel fortunate in that I didn’t have to add another med to everything I was taking. Everyone is different and for those who require these meds, it can mean the difference between finishing tx or not. I admire all who have endured tx more than once, Janet, as you have done. I don’t know if I could do it. I see my doctor a week from tomorrow for the results from the first tests since I ended tx. If there are any problems, I’m not sure what I’ll do. Now that it is behind me, I’m trying to pull the pieces of my life back together and if I had to face living through it again, I would not be happy camper! Guest guest
You are an exception Dorothy. Most humans could not endure the kind of emotional upheaval that you had going on when you went on TX. I know I struggle with a fraction of what you went through. That's why I say you are my inspiration If you can go through all this and come out the other end saying " it was not so bad " well jumpin jiminy cricket I sure as heck can deal with it!! Plus if I remember correctly you lived alone? That was dangerous and I am so glad it worked out for you. Heck just dealing with the emotional BS from menopause is bad enough. > > It's interesting since I feel that I was depressed to begin with.
