I know only a few who did not need an antidepressant while on tx BECAUSE the interferon depletes the brain of its seretonin.. IF you start tx without it, you may find some really really hard times and then if the doc gives them to you, you will still have nearly a month's time until they actually kick in... I know that Liz didnt need to take them, but she is one of the three I've known who made it through tx without them,, I know that I SURE WOULDNT DO TX without them and I do NOT have a depression problem either.. I started celexa while going through a really bad abusive marriage break up, but I've been off the depression meds now for 2 years.. but man, I wouldnt want to do tx without them,, but thats me..JackieSubject: Anyone not anti-depressants during treatment?To: Hepatitis_C_Central Date: Tuesday, June 24, 2008, 7:23 PM

I was already on them when I started treatment so I will probably continue on them during and after treatment. I just wish they would wean me off the Lithium already

My dr told me the very same thing......so therefore she didn't start me on one.....for me that was a BIG mistake on her part.....because I believe that in doing that....it caused me to have more problems.....once I talked her into giving me one....I felt SO much better....so from my experience I would certainly ask for one.....

I believe it is only used to treat

bi-polar disorder.

I am not on anti-depressants - mentally I'm feeling great. So not

everyone requires them. If you are suceptable to depression, I would

consider starting anti-depressants prior to treatment.

Believe me, besides stressing over the

shots, I had plenty more to be depressed about……..and I made it.

I know that many people have problems, but

it’s not written in stone that HCV treatment and anti-d’s go

hand-in-hand.

As long as they are not warranted, the

less meds you take, the better off you are.

I've known many many ppl who are NOT depressed at all who became nearly suicidal on treatment from the depression that interferon CAUSES... Really?  All 49,995+ needed

anti-depressants?  Well, let’s hope that massgal will be the sixth person

who doesn’t.

I think it’s criminal for a doctor

to prescribe anti-depressants to someone just based on the fact that they’re

starting HCV treatment.  If my doctor had suggested it, I would have refused.

Interferon

alpha (IFN-alpha) is a cytokine that is widely used for the treatment

of chronic viral infection or malignant disorders. During treatment

with IFN-alpha, severe neuropsychiatric syndromes may occur such as

depression with suicidal ideation, paranoid psychoses or confusional

states. The neurobiological correlates of these side effects are widely

unknown. Besides induction of other cytokines and hormonal changes,

IFN-alpha has been shown to modulate the opioid, serotonin, dopamine

and glutamate neurotransmitter system. Positive therapeutic effects of

antidepressants such as selective serotonin-reuptake-inhibitors (SSRI)

or of opioid receptor antagonists support the hypothesis that

neurotransmitter changes play an important role in the development of

IFN-alpha associated neuropsychiatric side effects. We review recent

Abstract

Interferon (IFN)-á, IFN-â, and IFN-ã are currently available for the

treatment of malignancies, viral infections (e.g., hepatitis C virus),

multiple sclerosis (MS), and skin conditions. In addition to their

therapeutic effects, IFNs commonly cause various side effects. Most

common among the side effects of IFN are “flu-like” symptoms such as

chills, fever, and muscle soreness. However, IFN can also cause

significant neuropsychiatric side effects, particularly symptoms of

depression.

A literature search was conducted in order to summarize current

information on (1) the frequency, characteristics, and risk factors of

IFN-induced depression, (2) possible biochemical mechanisms associated

with IFN-induced depression, and (3) the treatment strategies for

IFN-induced depression.

Review of the literature suggests that symptoms of depression

induced by IFN therapy, in particular IFN-á therapy, are common and can

limit the treatment utility, often necessitating discontinuation of IFN

therapy or the use of psychopharmacologic agents. Depression is also a

suspected side effect of therapy with IFN-â and IFN-ã; however, the

association has not been as convincingly confirmed. Importantly, IFNs

affect neurochemical pathways putatively involved in the etiology of

depression. While these depressive side effects usually resolve after

the completion of IFN therapy, they can persist or reappear with dose

escalations. It is recommended that health care providers, patients and

their families be informed about the potential risk of the psychiatric

disturbances that can occur with IFN-á therapy. Screening and

monitoring, ideally using symptom rating scales for depression, and

early antidepressant treatment intervention appear necessary to

Depression Due to Treatment with Pegylated Interferon plus Ribavirin May Be Under-diagnosed by Clinicians

The occurrence of depression has been reported in 20%-30% of cases that evaluated psychiatric side effects in chronic hepatitis C patients using pegylated interferon alfa plus ribavirin. However, the diagnosis of depression in these studies was determined solely based on the impression of the study investigators.

In the current French study, supported by Roche Laboratories, manufacturer of pegylated interferon alfa-2a (Pegasys), investigators assessed the incidence of depression during treatment with Pegasys plus ribavirin under “real life” circumstances using the Mini-International Neuropsychiatric Interview (MINI).

MINI is a short, structured diagnostic interview to assess psychiatric disorders that makes a correlation between the MINI results and the diagnosis made by a clinician and with the Beck Depression Inventory score (BDI).

The multicenter study is expected to enroll 150 HCV monoinfected patients treated with Pegasys plus ribavirin. The incidence of depression will be assessed at baseline, at weeks 4, 12, and 24, at the end of the treatment, and at the end of follow up using MINI, clinician observations, and BDI scores.

Interim study results from baseline through week 12 for the 123 first patients enrolled were presented at the 58th Annual Meeting of the American Association for the Study of Liver Diseases in Boston (November 2-6, 2007).

Results

Baseline patient characteristics were as follows:

Mean age: 48 years

Sex: 73 (59%) male;

HCV genotype 1: 67 (54%);

Advanced (stage F3-F4) fibrosis: 45 (37%);

Previously treatment-naive patients: 73 (59%).

History of injection drug use: 53 (43%);

History of alcohol abuse: 29 (24%);

At baseline, MINI revealed current major depression in 9 patients (7%).

7 received anti-depressant therapy before (n=5) or after (n=2) the beginning of Pegasys plus ribavirin treatment.

Between baseline and week 12, 21 (18%) cases of major depression were diagnosed by MINI.

In 16 of these cases (76%), clinicians assessed was no depression.

Conversely, in 7 cases (6%) the diagnosis of major depression was made by the clinician but not confirmed by the MINI.

Anti-depressant therapy was started in 17 cases (81%).

The correlation between MINI and BDI score was poor.

Conclusion

Based of these results, the investigators concluded, “The incidence of depression between baseline and week 12 was 18%.”

Furthermore, they stated, “Major depression [in patients using Pegasys plus ribavirin] seems to be under-diagnosed by clinicians.”

Finally, they concluded, “The systematic use of the Mini-International Neuropsychiatric Interview (MINI)…may be useful during peginterferon therapy.”

shots, I had plenty more to be depressed about……..and I made it. I know that many people have problems, but

it’s not written in stone that HCV treatment and anti-d’s go

hand-in-hand. As long as they are not warranted, the

less meds you take, the better off you are.

I went through 15 months of treatment , then treated 4 other times ( varying time lengths) and I never had to take an antidepressant . I have an excellent support system at home and at anytime if I started to suffer from severe depression my family would have told my doctors . So not all people have to take them , there are allot of mitigating factors

Quick question. Is their anyone here on treatment that is NOT onanti-depressants? I ask because I am starting treatment and my drsaid that I did not need to be on them because I have had no historyof depression. I guess I am going to see how I feel while on peg/rib and if I feelsad, etc then I will get a script for them.

I am on your side of this one Dorothy , but I also know a few people who went kinda wacko on treatment . I think allot of it depends on your outlook and support structure

Liz, YOU KNOW that IM NOT saying that everyone needs it.. YOU KNOW that.. but what I am saying is that there IS PROOF that the interferon DOES deplete seretonin in the brain and that can cause insomnia and depression.. Im NOT saying that everyone will get depressed, Im just saying that there IS EVIDENCE that the treatment does cause serious and severe mental issues. On the insert inside the Interferon box clearly states that it can cause homicidal and suicidal ideation.. WE'VE SEEN serious episodes of SERIOUS outbursts here on this forum as well as many of the other forums that we've been moderators on in the past. Why do you think we have this forum on moderation??? Because of this stuff in the past... All I was trying to say is that IT IS POSSIBLE and NEARLY ALL those I've had conversations with ARE on SSRI's and one must be aware of this possibility and address it should it come up. Wouldnt it be a shame for someone to find that they have to stop successful treatment BECAUSE of the depression that can be caused by Interferon. THERE is NO SHAME in needing an antidepressant while on tx.. Thats ALL Im saying and ALL IM going to say about this... I believe you honey , you don't need to sell me on this . My ex-brother in law committed suicide while on treatment . He was depressed before tx so it stands to reason he should have been on one during . Each patient should be looked at differently and all things should be considered . Interferon is a serotonin depeleter plain and simple and during treatment quality of life can be real hard on some due to the depletion .
