A very succesful therapy is also Nizoral 200 mg PO x 7 days. Also have her use a water/vinegar rinse of the external genitals after her daily shower. This mild acidity solution is very effective in preventing recurrent vulvar candida. The water / vinegar solution proportion is 10:1We even use it in my hospital for wound infections. For recurrent vaginal yeast infections

To help clear the bowel of yeast, which colonizes the vagina each cycle use

Nystatin 500,000 u tid for 2 wks #42

And

Clotrimazole vaginally (or equivalent) for one day after menses for three cycles. The yeast is in very low concentraions (a few million) after the period but builds up logarithmically to hundreds of millions or more by mid or end cycle. Nipping it in the bud prophylactically before symptoms is helpful

And

A good probiotic daily to improves intestinal (and concurrently) vaghinal flora such as Klaire labsTherbiotic women's formula would help also, but this is over the long term.

and

Have her reduce or stop consumption of refined sugar.

Dear list members,

I'm working with a 31yo G0 who has been having brown spotting for several days before her menses, and brown tail off as well. She has tried both oral prometrium during luteal phase and now I have placed her on progesterone vaginal suppositories. She is delighted that the brown spotting has stopped, but during each of her cycles with supplemental progesterone, whether oral or vaginal, has had trouble with monilial (yeast) vaginitis occurring around day 9 of supplementation. She never had trouble with monilial vaginitis ever before this. This last cycle she treated herself with an OTC preparation which promptly relieved her symptoms.

I've never had a patient do this--is it usual, or any advice on how to navigate? She wants to continue using it, but is disturbed by the yeast infections.
