I'm 42 and never had a sinus problem before, let alone heard of sinusitis until last summer when it started as what I thought was a cold. I live in Northern NJ. I've never had surgery. Anyway, for whoever is or may get needle aspiration to drain a closed maxillary and the doc doesn't get it cultured, he is an inconsiderate quack, run for your life and get someone who will isolate the pathogens on antibiogram for resistance/susceptibility. Thanks, PS all, I think it is worth noting that actinocycosis may be viewed by a lab as a contaminant since it is a normal sinus flora. BUT, if it comes up in a culture from needle aspiration of the maxillary, it is not a contaminant because the sinuses past the ostium are supposed to be sterile. Insist your doctor tell the lab to report all growth without adding inhibitors and keep the growth going for a few months. If I had it to do over I never would have consented. It did not cure it. Looking back I think they insisted because I had polyps and had a hard time breathing. They didn't know as much as they do now. If I knew what I know now I would insist on a culture. If you don't the bugs can become resistant and you will have a hard time healing. Also I would go out and get a delivery system for irrigation and use a product called Breath Ease to rinse your sinuses. There is a product that is like a water pic (you can use a water pic). I use a Neti pot which I found a long time ago in a health food store. I like it better than anything I have tried. I live in Sacramento, CA so it wouldn't be possible to travel to NY for txment. I have used Gentimysin compounded in a saline solution which didn't help. I just got an Rx for Tobramycin that is compounded for a nebulizer. The second time I used it I broke out in a rash. That happed Thurs night and I didn't get the Dr. called yet. I am taking perscription Niacin from another Dr. and after reading I think the rash may have been caused from that as I had a repeat rash yesterday and had not taken the Tobramycin. Tomorrow I will be calling both Drs. to see if I can proceed with the Tobramycin. I am also on a multiple antibiotic therapy for Chlaymidia Pneumonia. It is protocol coming from Vanderbilt U. You can read about it on Cpnhelp.org. Thanks again for the reply. I will go to the website you suggested. My ENT Dr. has suggested that if the nebulized med doesn't work he may order IV antibiotics. He also talked about more surgery which I am very reluctant to do as it has never helped before. He was given every ABX known to work on sinus infections and they cultured itn 3 or 4 times, never came back with anything. I wonder if it wasn't because they weren't looking for the right thing....like yeast maybe. He had 1 sinus cavity infected and by the time they did surgery 7 months later it was 3 sinus cavities. They even tried flushing it out by shoving needles through his face. I knew several people during that time period that had infections that ended up needing surgery. The end result is a head full of scar tissue and celia that won't self clean your sinuses anymore. Also I think I contracted more infections on the table or in the office getting suctions, dx procedures etc. I am a veteran. Four surgeries. Have turned down as many surgeries as I have had and they want to do more. Surgery has never helped me, not one bit and I think has made the problem worse. It protects the skin against other pathogens BUT what happens in many of us with dysfunctional immune systems is we develope a problem with Coag. Negative Staph Spi. When a cuture is sent to a lab they thrw away aculture that grows any staph epi, assuming it is normal flora. There is a specialty lab that deals with culturing coag. neg staph epi. It is called Espterix Lab. But the doctor has to specifically ask them to please look for Coag negative staph epi. This is never a pathogen for those with intack immune systems but for those of us with dysfunctional immune systems it can cause great pain and suffering. I know! I've had to deal with it. Many have had surgery, therefore whatever they once had on the sinus mucosa surface is now disseminated throughout tissue (all that melting pot of flora), having now become a polymicrobial soft tissue infection caused by the very surgery performed to cure fungi that could have used some amphotericin. Then you have the people who didn't have the surgery, but have had sinusitis so long, maybe from the fungi, but now after many months or years of immotile cilia have invasive polymicrobial soft tissue bacterial infections, they having breached the mucosa, the first line defense. So, for people like who have shooting pains through the ears, or pain on the skull, fatigue, etc., vs. stuffy nose with sinus headache, there is likelihood that their condition is more than advanced past a stage where flushing with or even IV amphotericin B would cure. I think the first step should be typical cultures. If that does not work, a thorough quest for the atypicals that require months or years of abx such as BB lyme, mycoplasma, chlamydia, etc. EJ mentioned coag neg staph. Somehow, responses to someone named Marie became involved with my question. My current question is about concha bullosa, how they can affect sinuses, and what happens after they're removed. If I have this fluid drained will it disappear. Also, not being a biologist, I don't know what all of these bacteria are that are being referred to. It seems a little obvious though, that since this fluid started during the sinusitis, it's some type of bacteria.
