I suggested at the Sacramento conference that if a child responds with hyperactivity, DMG plus folinic (DMG uses folic acid in its metabolism) would support nucleotide synthesis side of folate metabolism but will not help methylation directly. Most kids can tolerate TMG and since DMG is the product of the TMG methyl transfer, you get two for the price of one - but TMG should not be used if it causes hyperactivity. The folinic with TMG is to support the DMJG that is produced with TMG metabolism. " Clinically I find some kids tolerate DMG, some tolerate TMG, some tolerate both, and some neither. However, since Jill's work I have tried to get most kids on TMG even if I have to start with lower doses and work up, with plenty of folinic acid, but still there are some who just cannot handle it, and if they can handle DMG, I have them on that, as I have always found that beneficial for many of my patients (and they like to take the little sublinguals).
