Guest guest Posted September 23, 2004 Report Share Posted September 23, 2004 I have had patients come to me from other physicians and they ask me to get their old records for them from their previous doc...my HIPAA manual provides me with a form to release MY records if a patient so desires, but does anyone have a form that allows you to request another physician's records for a patient (with their signature, of course)? I try to get them to go to the old office and sign THAT doctor's release form, but many patients seem reluctant to do this.
