RE: questions about start up

The site review I had focused on quality of practice questions: "how long for the next appointment, how long to wait for an appointment for a physical, acute visits, access to care, quality of record keeping." The nurse doing the site review sent in her family to establish with me after the review, so no problem with an "atypical" office.

1) Is there a problem getting on insurance plans?

This is probably quite variable among different geographic localities.

Most hospitals will have a Medical Staff Organization and can help you assess

the situation. AlWAYS assume the entire credentialing process will take 4-5

months. also some county medical societies can help you get the scoop.

while you are starting your practice and are slow to begin with..it might be

tempting to sign up with multiple insurance plans. DO NOT DO THIS. many plans

have contract language that is not fair to the physician. in many instances the

big plans (CIGNA, BLUES..etc..) will require you to take on a minimal of several

hundred patients. the administration of multiple plans will be impossible for

you to manage without hiring expensive fulltime help.

alternative income sources in the first year that should be considered.

1. State Disability exams.

2. Part-time Locum tenens in your same county.

3. Teaching med students,residents or PA's.

4. make it a point to personally meet as many physicians in your commnunity

as possible. Opportunities often arise . When first started I met one

of our local psychiatrists who needed help with History and Physicals for

a busy inpatient psychiatric unit. I must have done 12/week for 6 months

2) Is there a problem passing the site inspection with an office that

looks atypical. The reason i ask is that most of these people are

beaurocratic RNs who take a very superficial look and i wonder if the small

offices scare them off?

shouldn't be a problem....but office location/looks is most important for the

first year of practice...as you get esatblished it becomes less of an issue

3) Should I join a PO? YES....PO's can simplify your applications.

I had no problem getting on insurance plans or passing site inspection with a

very small low overhead practice. It takes about 5 months to get

credentialed/contracts signed etc. I am still waiting not so patiently for my

first check (it took an extra 60 days to coordinate webmd with the individual

insurance companies).
