Here is a website that already exists for this purpose. Has some great forms on it. I haven't updated it in a few months. But I too would be happy to store stuff on it.

I am attaching a revision of this spreadsheet. The new one actually

uses the retainer fee to calculate how it affects how many (or few)

patients you need to see a day to break even, whereas the old one

didn't. Plus I uploaded a copy to the group website. Hope this helps.

That reminds me, I better back up my files again...

Hello Gordon, I love the idea of a form library, I would be glad to

send you forms however I am not smart enough to get them out of the

computer to this site. I could fax them to you out of Alteer if you

could give me your fax number.
