We have a patient that lives here in Arkansas but has been to Baylor for work-up b/c UH was primary; now that MC has become primary, pt has switched to us b/c he doesn't want to stay with Dallas (sorry Amy) and its easier for him to stay in Ark. I've gotten his benefits and they will be OON; now.. 1. do you guys ever type up the Ins benefits and have pt. sign off that they agree??? or 2. do you just verbally go over? If you have them sign off, would you you share your form with me?? We had it printed in triplicate to keep a copy in the pateint's chart and financial file. I will send it to you privately.
