To enter a doc into our database:

Log onto our site.

Click on "Database" in the menu.

Under "Tables" click on "Doctor Reports."

Scroll sideways. Click on "Add Record" above the existing records.

Fill in each of the blanks.

Scroll down and click on "Add Record."

You are done.
