My annual fee form

I have gotten a couple of requests to see my form for my Annual Non-

covered Benefits Fee (aka Admin fee) so I am sending it the the

listserve as an attachment in case there are others interested in

modifying it for their own practices. As I have stated before, I don't

call it an administrative fee because I was told that insurance plans

consider any administrative costs to be bundled into their

reimbursements already. If you call it an administrative fee, don't be

surprised if insurance plans tell you that you can't do that if they

ever find out about your fee.

( & others who charge an annual fee),

Isn't it sort of a pain to collect these annual fees?

Do you have to keep reminding patients to pay?
