You can print off the form off the SSA website. Make sure you give this information on that form - add another page if you need to: http://www.ssa.gov/cgi-bin/formsearch.pl YOU NEED FORM #SSA- 561-U2 It is a pdf file so make sure you have Acrobat Reader to open it. " No matter what kind of disability benefits you are applying for, you must give us information about your medical, work, and education history to help us decide if you are disabled. " - from http://www.ssa.gov/d & s1.htm Second to that, yes, possibly your age is a factor and Voc Rehab retraining may wish to retrain you for another job, and thirdly, because of your young age, you may not have enough years of work. But, it is difficult to know where the 'hang up' is without seeing the entire contents of your file and the rulings. I would still consider finding an experienced attorney through Nosscr - it won't cost you anything to speak with her/him and help get your paperwork straight.
