When it was time to go to toe shoes, my instructor told me and my mother that I would have to stop going as high on my tiptoes or I'd never stay in the toe shoes. I was always proud of my ability to stand on my tiptoes -- what I was doing was standing on the top of the ball of my foot and my high arch would jut out over the top...if that makes any sense. The instructor didn't know that I had problems...heck, neither did I or my mother. But I realized then that I'd never be able to go into toe shoes. The instructor thought I could simply modify the way I stood on tiptoe...I realized that that WAS the way I stood on tiptoe and wouldn't be able to do anything differently. I think you should discuss the CMT and future issues with your daughter so she understands that she may have to alter her dreams of being a ballerina. That occupation is just too hard for someone with CMT, IMO. It's better to start her thinking in other directions before she gets too far along.
