IT SHOULD BE A MEDICATION PERMISSION AUTHORIZATION ONLY, I DON'T THINK YOU

HAVE TO HAVE A NOTE STATING WHY THE MEDICATION IS supposed TO BE GIVEN , JUST

THE AUTHORIZATION FROM DR. THAT IS ALL I HAVE BEEN TOLD AND GIVEN SINCE MY SON

HAS BEEN ENROLLED IN SCHOOL FOR 5 YRS.

This is interesting because up until now, I've never had to provide a doctor's

note. I've always filled out the sheet to accompany a new prescription for

stimulants, asthma medication, whatever, but I've never had to provide a note

from the doctor to explain WHY my kid is taking this medication. I always was

the only one to fill in the blank: condition for why medication is prescribed.

At the middle school and at the other elementary school, I just fill out the

sheet and my signature has always sufficed. No questions asked.

Interestingly enough, the pharmarcy filled the prescription without questioning

the pdoc. I just hit the brick wall with the nurse.
