Guest guest Posted October 18, 2004 Report Share Posted October 18, 2004 IT SHOULD BE A MEDICATION PERMISSION AUTHORIZATION ONLY, I DON'T THINK YOU HAVE TO HAVE A NOTE STATING WHY THE MEDICATION IS supposed TO BE GIVEN , JUST THE AUTHORIZATION FROM DR. I've always filled out the sheet to accompany a new prescription for stimulants, asthma medication, whatever, but I've never had to provide a note from the doctor to explain WHY my kid is taking this medication. I always was the only one to fill in the blank: condition for why medication is prescribed. At the middle school and at the other elementary school, I just fill out the sheet and my signature has always sufficed. No questions asked. Interestingly enough, the pharmarcy filled the prescription without questioning the pdoc.
