You need to contact Plass. I can send her email to you... when I

get home. I am watching my friends kids today and not home.

send it to you while I am not home.

You will need to complete some paper work and submit PERTINENT medical

records along with a letter summarizing the symptoms by ped or a

specialist. Once submitted you wait... and wait! <smile>

It can take up to 6 months but on average 3-4 months for a call to set

up appointment!! presents the medical records and letters to

Kastner for review... then she contacts you.
