Pre surgery evaluation

For those of you who see patients as part

of the pre surgery evaluation, how do you determine calorie levels? Do you put

patients on a strict diet that mimics what they will be on post surgery? How

frequently do you follow up prior to surgery if surgery is 4-6 months out?
