Guest guest
Are you required to use BOLD database if you are a center of excellence? If not, how are people planning on submitting annual data to SRC? We are looking into a new database. I know BOLD is good but our facility is providing us with a free system. Also, we require weight loss prior to surgery. Is anyone at a center of excellence that does not require weight loss? One of our practitioners would like to not require weight loss. I know it doesn't determine success but I think it's a good way to get people to evaluate what they are eating. Any thoughts? Thanks,
Guest guest
Everyone must use BOLD as part of the agreement as COE. You can use your own database as well and can "dump" data into BOLD regularly. They are working on being able to interface with most other databases. We are a COE and do not require a particular amount of weight loss. We do ask that patients either maintain or lose some weight during the supervised weight management program. We document compliance via food journaling and making lifestyle changes. If we see drastic weight gain, patients are changed to one on one appointments with the dietiitan.jdm Janelle D. Guest guest
On the topic of pre-op weight loss, the surgeons I work with require 5-10% in order to shrink the liver. Also, by having a patient on the downward trend, there is likely less of the inflammatory effect caused by obesity. So in addition to helping increase awareness in the patient, pre-op weight loss is also helping to decrease the risk of complications. Guest guest
I am really interested in your responses on this - I am looking at building an Access database -but need the data markers so I know I am collecting the right information. Guest guest
We are a COE and we have been using Lap Base for our data. We talk about adapting to BOLD one day, but is is expensive & I do not think we will do it until it is mandatory. We DO NOT require weight loss prior to surgery (surgeries= Band, RYGB, & DS). We only ask for weight loss on the following patient types: Liver size exceeding...??... (I forget the size/I'm at home) which is discovered on their abdominal ultrasound If their weight exceeds 450 lbs. BMI > 55 for males who want the DS (usually we will not do the DS for this pt population) Or if the weight is a bit lower (say 425 lbs.), but their body habitus is such that the docs KNOW pt will be a difficult case in the OR. I'd say < 5% of our patients have to go on the liquid diet.
