From the 2005 Chiro Code Desk Book 97140 Bundling When using a physical therapy procedure such as 97140, there are three things that should be documented: 1) technique (manual traction, myofascial release, mobilization, etc.),2) location (cervical, lumbar, shoulder)-this helps show a different area when used in conjunction with the CMT code, 3) time. For 97140 use with CMT, append the modifier-59 (97140-59). it could also be helpful to explain in box 19 " at a different area than the CMT adjustment/manipulation, " etc. Note new address! W. It is a new ploy to reduce cost and probably a shadow of the workman's comp law from 2 sessions ago. Don't know whether it is legal though. I seem to remember that it was the tx fees NOT the procedures that were tied to that legislation. Vern or Freeman may be able to shed light on this! Progressive has just denied the 97140 saying it had to be done on separate regions then the adjustments even though I used the "-59" modifier. I think I remember a while back a discussion on this list involving this issue. I also remember some AMA ruling that these 2 procedures could in fact be done and paid for if done in the same region! The following is from page 93 of the 2005 Chiro Code Desk Book… “ How Long is 15 minutes? Historically, for decades " 15 Minutes " was accepted as up-to 15 minutes. However, in recent years more definitive approaches have transpired by payers. In 2000, in an effort to control fraud and abuse of costs, CMS and adopted a policy of rounding to the nearest 15 minutes. This is a logical and simple concept. Accordingly, 8 to 23 minutes of hands-on care would be considered as one 15 minute unit of care, 23 to 38 minutes would be 2 units, etc. When more than one service code is performed, the time is bundled; and the total units cannot exceed the total hands-on time rule. Other policies might vary, but this CMS approach is inherently reasonable. If less than 8 minutes (1 unit), it would be appropriate to append the modifier -52 Reduced Service, and charge less accordingly. If adjusting cervicals, you could bill 97140 if applied on thoracics. Still apply the -59modifier. A note explaining code billing accompanying the bill has reduced denial of all kinds for me.
