Athletic release form

I am writing to see if anyone has access to an athletic release form that I

could use for high school sports. I want to help out with HS football team but

I need a parental release form to protect myself and the school.

If anyone has knowledge of this please give me call or email me.

Will

There is a standard form used by ALL schools. You need to go to OSAA.com

Might be .org and down load and print. Front has Hx on it and needs to be

signed by parent or gaurdian, the back is the physical part. Ther are also

" recomended " guidelines to perform the physical. If youi can't access then

let me know I will fax it to you.

Call Palmer college... They have an athletic trainer there who can send you the form they use. I used to have a form that they used...I can't find it now. If you are going to be on-field.....it is a pain in butt to have a form on a case-by-case basis...I have done road races and bike rides etc and this is a different situation... for sports like football/soccer etc you are better off signing an agreement with the district attesting that that you are licensed and insured etc...also it is 100% key that everyone is clear as to what your EXACT role is vis-a-vis any other health care pros at the event....i.e. is there a clinical chain of command? what is role of chiro vs the trainers vs the MD (if any) vs the EMT personnel on-hand. This has to be 100% absolutely clear with all healthcare personnel if you want the scenario to succeed. Even if you don't have a form...if you have a verbal consent from the school's A.D. and have met with the coach and all other healthcare personnel you should be good to go as now you have "implied consent".
