voice recognition software??

Hi Marc,

I am on Naturally Speaking 7.0 w/ a fairly fast (half-fast?) box. It works

OK, is faster and cheaper than voice-to-tape-to-trascriptionist to chart. It

takes time, but when its done, its done, signed and in the chart.

I am curious whether voice recognition software has really matured enough to

use for dictation/transcription??

I tried Dragon a few years ago, obviously with a slower computer, and was

very frustrated with it.

Has anyone been happy with it. Is there a particular software that you like.

Dragon's NaturallySpeaking, professional version 7.0 at least.

I use it on a Dell laptop. Definitely way more fun than typing! Not 100% accurate, so you have to keep your eye on the text as you dictate and make manual changes every few sentences. But it is definitely much quicker than typing , especially for a slow typer.

I have looked at this technology repetitively over the years but it has not gotten to the point where it can replace my transcriptionist, which is a real pity because she makes quite few mistakes. I have just heard of a transcription service in India where you send over your digital files and apparently they do quite a good job. I will report back if this turns out to be a good thing.

I would encourage you to look again at Dragon version 6 or higher. I have not used a transcriptionist for 2 years now and the technology is way beyond here.

Since I bought a laptop last November, I have it in in the room with meon my lap and type everything the patient says and that I do. Sometimes I have to make some hand notes with involved exams and finish it later. All but first visits are almost always done before I see the next patient. Cheap technology, total control of the final product.

You are absolutely right - it does seem provincial (couldn't resist). It would probably take me 4 or 5 times as long to type my reports as to dictate them (I am not typing patient notes - the laptop idea makes sense for that - but rather 5 -15 page reports). I am going to take Coby's advice and check out Dragon 6 again.

I have been considering same sort of idea. Do you have more than one tx room? If so, do you tote laptop with you, or is it always in room? If so, what about privacy? Do you log on and off between each patient? (Are they ever alone in the room without you, and WITH the laptop?) Trying to figure out a practical solution. Would LOVE to go to typewritten notes- my handwriting isn't that great...

I have pondered the voice recognition/transcriptionist question and come up with a solution for my own needs:

I have a private consultation chamber where my server is located (password protected). I personally type better than I speak, and think faster than I type, so the luxury is to make notes useful and as brief or extended as needed.

I also have a low enough patient volume that usually there is no problem in timely chart entries.

I did use a court room transcriptionist for a number of years; she eventually retired. She supplied me with the dictation gear that worked for her setup. It was not too expensive, and her work was high quality. This was before HIPPA.

In spite of the alleged benefits, every time I add another layer of technology to solve one problem, two new problems arise, and I find new reasons to cast Bill Gates into the 7th ring of hell. Just my humble opinion.

I'm considering going back to the 8088s and CPM. Or maybe Apple. Or just maybe PAPER and PEN. There remains so many glitches, unresolvable, in IE, Money, and Windows version Whatever, my time spent without the obnoxious obtrusive demanding and otherwise frustrating affgects of software/hardware/ISP/"the Net"/etc is so pleasant in comparison, I won't miss it.

I've been online and computerized from the days it was a giant leap forward to have a 5 meg hard drive....clock speed jumped from 4 to 8 Hz and then 12 to 20 Hz.....I ran my office for years on a 20 Meg hard drive..........

I have looked for the holy grail of chart noting. I've tried palm based products (patient keeper), palm/pc products (auto doc), and of course, pen and paper. Nowadays I utilize a central laptop and chart via keystroke in Word in between patients (with a few macros and boilerplate files I've created). It isn't a perfect system, but I too type better than I write and it is nice to come back to a patient file 6 months or 1 year later and actually be able to understand what I wrote!

PS. Do you guys get requests for records "including all handwritten notes" from 3rd party (primarily) pip? If I have handwritten notes at the time of exam I transcribe to my PC and shred the handwritten.

sounds like your setup is the closest to what I am envisioning might work for me. Sounds like you are adept at creating Macros.

You might be adept at creating Macros. Do you ever draw pictures? I tend to make marks on the "Universal Man" to illustrate areas of focus in treatment. These are a great visual tool for me in reviewing the file later, or just for visit-visit comparison. Is there any possibility of creating that sort of system in the Word notes you keep?

are your Word notes linked at all to the billing for these patients? Do you even pull charts for patients on each visit, or do they only get pulled if something else is needed? Do you print out these typed notes each time, or what exactly? So many questions, so little time!

I am wondering a bit whether these typed notes need be always in contemporaneous hard copy, or can entire patient charts be kept on computer and spit out only upon request?

I'm tickled to have sparked an interesting discussion, today, discussed voice recog with my computer guy, he says that of his clients who tried VR, who include about 6 MDs, none have continued to use VR. They all stopped at dragon 6, I don't know if version 7 is significantly different.

I can get dragon 7 on ebay for about $150 or so, and I'm going to see if I can buy or download a demo version or returnable version, and see if I can make it work. The private space issue is an interesting one, HIPPA has sure made our lives more complicated.

Dear docs,

I have been researching this area for quite some time now. I have decided that the best electronic charting software for the money is WritePad. They gave us a trial CD to use and it is very impressive. You can customize EVERYTHING. You can also have it call up different sets of exams/questions for different doctors in the clinic. You can even make your own templates if you want. It is also compatible with Dragon, and will interface with most billing software (not Medisoft yet, they are working on it). The software has a chiro specific module so almost anything you can think of is in there. If it's not, you can make it yourself. We do not have the software yet, but we are working on getting it. It is also compatible with any hardware (laptop, desktop, pocket PC, tablet PC). It is also fully HIPPA compliant. The sales dept. responds promptly to your emails also.

One copy of the software is about $2500.00. Each subsequent license is about $750.00. Each doctor in the clinic needs a licensed copy, but that doctor can put that software on as many PCs as s/he wants. That's pretty cheap compared to a lot of others out there. They also supplied me with a list of chiropractors across the country using the software. I called about 1/2 of them and they all RAVED about it.

All the new technology is allowing so many choices today. It can be confusing.

As a member of the Seed Panel for the Record Keeping chapter of the Educational Manual for Evidence-Based Chiropractic, we have just completed statements about just that. With the evidence, what we found is that, as long as the proper safety mechanisms are in place for patient confidentiality, keeping patient info on a hard drive is up to the practitioner. The one caveat is that all sources of data must be provided when properly authorized data is requested to be released....meaning you just need to be able to find all the pieces and put them together when data is requested. Having a statement in any paper file about where to find the clinical data and notes would probably in order.

Sorry, Mike, that wasn't quite clear to me. You used Dragon 6 and it was terrible. YOu went back to Dragon 5 and are now attempting Dragon 6 again??

When your colleague uses the "pad", what sort of equipment is that? Does she type onto it, touch various pre-recorded sentence bits (like the bar-code type that was out 5-10 years ago), or ???

I corrected my first e-mail............... I am loading Dragon 7 today. 6 did not work and the company admits it. I am currently using 5. I also found some software that will "boilerplate" forms that you can tab into and use Dragon. I am finding out more about the system my colleague is using later this week. I know she uses boilerplated bits that form fairly complete sentences.
