I would avoid using any forms not supplied by the company, as they range from completely insufficient to well over-the-top. Without knowing exactly what the dive contractor requires, you're opening a can of worms that you don't have enough insurance to cover. If they have put it on you to develop (that scares me BUT) punt it to your/their medical director, who will pull a form out of his <whereever> and use that. If your/their medical director doesn't have enough experience in diving to have a dozen forms already, you need to find a better job or a better doc. Don't let them give you a " diver medical exam " sheet--they take -forever- to do, you aren't qualified to make some of the calls, and they only need to be done on an annual basis for most places. If you're not worried about liability at all (I'm American, obviously, and we're sue-happy) then there are a bunch of decent forms that cover the basics. Don't get yourself bogged down in minutiae--it's a waste of time. You'll probably be under some pressue to get the guys in the can as soon as possible, it's likely to be 0230 in the morning, and these guys are professionals at hiding anything they don't want you to know about (and many will.) But, I want to add some more insight. I worked on a dive ship, and this was my favorite job, of my career. Even, over the fire dept where, I retired from..... Remember the company wants to honestly approve a diver to work. Should a diver become ill while in saturation, the project is prolonged and the profit ratio also becomes more narrow. Also, they don't want divers disqualified, over every little thing. Like A.Austin said....If you have to deem a diver not fit for saturation, make sure to document, your doctor and/or the dive company's doctor is in agreement. I spent 30 minutes doing a head to toe exam. Checking for skin infections. Evaluating the mental status, checking for mental depression and suicidal thoughts. Checking the teeth, gums and ears. The company (Dive Supervisor) told me, " the last dive medic took only 4 minutes per pre-dive exam. " So, be sure to have a honest and diplomatic reply. In 6 hours, I could examine 12 sat divers..I usually started at midnight, and had 8 to 12 divers ready by 6 AM. During this project, I failed only 3 of about 50 divers..one was drunk, one had a skin rash and the other, an abcessed tooth. Good luck with your endeavor...and remember the dive manuals are a wealth of information. Guest guest Posted June 25, 2009 Report Share Posted June 25, 2009 Ricky, Listen to what these guys are saying. Dive medicine is a training intensive specialty. Specialties are specialties for a reason. If you do not have DMT, chamber operator, or other equivalent dive medicine training you should seriously be thinking about a new post. Clearing divers in and out of the can is not a place for on the job training. Of course you have to do what is best for you, and you are the only one with all the details, but the purpose of a board like this is to share and benefit from each others experience. Ricky simply asked if anyone had pre and post sat medical forms they could share, he did not at any point mention why he needed them, what he was up to or what his professional background is, he could simply be drawing comparisons or doing some research…. I don’t really feel there is a need to suppose he is up to anything which is a either unethical or b unqualified for. Therefore suggest maybe this thread is dropped…..
