If you are homeschooling the kids, why do you need to file an exemption form

with the department of health?

Are you talking about the religious exemption form at the Health

Dept.? I am from TN and I homeschool so all I did was file this

exemption form from the HD, notarized in my childrens records. I was

not aware that you had to add anything extra to this form. Have you

been told otherwise?

everyone - you have to USE THE EXACT wording of your law.

I don't know anything about a 'green sheet'

Here is the law

" Requirements do not apply to any " child whose parent or guardian

files with proper authorities a signed, written statement that such

immunization and other preventative measures conflict with the

religious tenets and practices of the parent or guardian affirmed

under penalties of perjury. " "

You just say " such immunization and other preventative measures

conflict with the religious tenets and practices of

______________________ (the parent or guardian).

PERIOD - that is it - nothing more - don't explain, don't embellish

attach a copy of the law if you want - found at above webpage

I dont know about the form at the health dept. I didn't know there was one.

This is the green vax record sheet they need for schools, sports, and other

things. It has a mark on the bottom that states " religious exemption " and then

states it should be attached and signed. Where did you get this form? Do I

have to go to the health dept to get it? Where is that-is there somewhere in

most towns I can go for it?

As for homeschooling and needing the form, I think its better safe than sorry.

Should something ever happen and I need my child in a school or any other

situation, I want to have this already on file. Besides, some public sports

teams and the such require it.

Thanks Sheri. I will type this up and sign it and have it scanned into his file

at his ped's along with keeping it on file here and a copy within our last will

and testament!

I'm a little confused. I've never filed an exemption with the Dept. of Health.

Exemption forms are for school/daycare and it goes to them, not the state. Is it

different in TN? I'm in CT.

FYI--I never needed an exemption form for sports. Cub Scouts required one but

they have their own form, strangely enough. I include them for camps which use

the same health form as the school, but I don't know if they actually require

them. Might depend on the camp.

Some states require an exemption to be filed even if you homeschool.

In Colorado homeschoolers are required to file an exemption every year

with the school district in which they reside. You have to

investigate and know the law in your state.

Yes, I have to use an umbrella school to report grades which requires

immunization records upon registration (don't know why exactly). So, I

sent a letter to them exempting out of vaccines for religious reasons

to keep on file. Later, I just picked up the typed up forms from the

health dept to have signed and notarized in my personal files just in

case I was ever asked for something more " official " .
