I am from TN and I homeschool so all I did was file this exemption form from the HD, notarized in my childrens records. I was not aware that you had to add anything extra to this form. This is the green vax record sheet they need for schools, sports, and other things. It has a mark on the bottom that states " religious exemption " and then states it should be attached and signed. Where did you get this form? Do I have to go to the health dept to get it? Where is that-is there somewhere in most towns I can go for it? As for homeschooling and needing the form, I think its better safe than sorry. Should something ever happen and I need my child in a school or any other situation, I want to have this already on file. Besides, some public sports teams and the such require it. Exemption forms are for school/daycare and it goes to them, not the state. Is it different in TN? I'm in CT. FYI--I never needed an exemption form for sports. Cub Scouts required one but they have their own form, strangely enough. I include them for camps which use the same health form as the school, but I don't know if they actually require them. In Colorado homeschoolers are required to file an exemption every year with the school district in which they reside. You have to investigate and know the law in your state. Guest guest Posted January 19, 2009 Report Share Posted January 19, 2009 Yes, I have to use an umbrella school to report grades which requires immunization records upon registration (don't know why exactly). So, I sent a letter to them exempting out of vaccines for religious reasons to keep on file. Later, I just picked up the typed up forms from the health dept to have signed and notarized in my personal files just in case I was ever asked for something more " official " .
