I went for a long trail ride yesterday on Red. He's a red roan quarter horse that I am leasing this month.

Well, in order to stay on topic, this is what I noticed. After cantering (similar to a moderate gallop) quite a bit, I went home and was hungry. I was able to eat a piece of pizza like I was normal.

I was wondering if the stuck feeling is food accumulating beyond what is normally stuck in there that I can't feel. I thought that maybe the bumping motion cleared most or all of the food out of my esophagus and I could eat "normal" for a while until the accumulation started up again. This has happened a couple of times recently as I have became a better rider and can go faster.

Has anyone else noticed jarring movements that clear the esophagus, such as roller coasters, carnival rides, bungee jumping?

Another thing they harped on at the conference is that a patient's subjective reports were NOT a good indication of esophageal emptying. People would say they were doing "much better" and when they did a timed barium swallow (TBS) on them, they showed WORSENING of symptoms compared to the last time.

Dr. Rice said he likes to see his pts. yearly after a myotomy to make sure things are still going well, rather than relying on the "call me if you need me" approach. This way, they can go in and dilate/reoperate before the esophagus stretches out again. I believe Dr. Hunter said he was going to start doing the same thing w/ his surgical patients (previously he had been of the "call me if you need me" mindset.)

They didn't get into WHY this happens, but I tend to think along the lines that you do -- that our nerves are so screwed up that we just don't KNOW that we have retained food until it works up far enough to where we do still have sensation.

Hi Pete,

I know exactly when the food goes down and I'm able to resume eating. I actually feel it, and with a sigh of relief, I am able to continue. I jog, and after a good run, that's about the only time I can drink quickly. Also, after a gastroscopy and cleaning, I am able to eat much more before having to stop. I'm still doing fairly well after my last one, and it was several months ago. It's not in your head :-)

I just spoke to a nurse about all the milk I drink. (She had never heard of Achalasia) I drink a litre every day. I was hoping it was helping to clean everything on the way down. I was told that the milk was coating my esophagus but probably sitting too long and possibly turning into acid. I'm on Nexium to reduce acid. Any advice from all of our milk drinkers? I was quite concerned when she told me this. I've been drinking milk for years.

P.S. If I went bungee jumping, my esophagus would clear alright! Just in the opposite direction! :-) Look out below!!!

, at TCC they said that "heartburn" in achalasians is likely due to LACTIC acid from stasis (retained food decomposing), not GASTRIC acid from reflux.

I would suggest using something like Tums/Rolaids/Mylanta/whatever to neutralize acid in your esophagus -- it will "stick around" along w/ the retained food. Nexium and its counterparts are designed to be absorbed into the bloodstream to then "shut off" the acid-producing cells in the stomach. If your acid isn't from stomach acid refluxing, then Nexium isn't gonna do a darn thing -- you don't need to shut off the stomach acid, you need to get rid of the acid that is already in your esophagus.

Then again, if you DO have refluxing acid in your esophagus, Nexium may indeed help.

That's probably why taking a ranitidine as soon as I feel the dreading burning works right away for me. Yet, I haven't had a full blown spasm since I started taking Nexium every evening at 7:00. I don't eat after 7:00. I'm going to switch from milk to water and see if I notice any difference. My Doctor said I have a lot of strictures from acid refluxing in my esophagus. Look what happens when you "think" you're doing the right thing. :-(

He knows I drink a lot of milk and never said a word about Lactic Acid.

in cases like mine. I have had one dilatation,

but the opening is becoming smaller and I am

regurgitating more food as it does. I have found

that the first meal of the day goes down and

stays down a lot easier. I have assumed that

the time between is long enough that the other

foods have had time to work through.

It certainly sounds reasonable to me that the

jogging would help the food to go down as well.

Others have reported that jumping up and down

helps their food to go down.

always been one of the “jumpers” …but not the bungee jumpers.

(I agree with that bungee jumping would be really disgusting for the poor

people standing below!). LMAO

What other way can we jar our esophageal contents?

How about slam dancing, pogo stick jumping, bull riding, bronco bucking, moon room jumping, parachuting, ski jumping, skateboard parks, roller coasters, etc.

One thing to keep in mind, too -- at TCC they talked about one cause of NCCP's being related to a HYPERSENSITIVE response (by the brain) to a NORMAL stimulus. It's possible that NORMAL acid levels in your stomach causes a HYPER reaction in the brain, and so by reducing the acid in your stomach you have reduced your NCCP's. Could be completely and totally separate from "retained food" acid... you may need an "antacid" (neutralizer like Tums, etc.) for the "retained food" situation and a PPI (like Nexium) for the stomach acid.

Sure are plenty of different "angles" to this disease, aren't there????
