You should not have to do anther appeal. All you need to do is fill out a

claim form. Make sure you keep copies of every thing you send Cigna. I know

and had my surgery 10-30-02. Yes they are paying but barely. Cigna denied one

of the procediures (code# 43846 ) their reason for denial was verbadam is One

of the billed procedures has been denied because it is not typically

performed on the same date of service as the other billed procedures. Good
