You asked:  Also, someone posted about keeping a daily journal.  What

do you note, as I think that would be a great thing to have with me.  I

have been running a low grade fever and feeling generally crummy for

several months now and it is starting to get very frustrating.

My suggestion: I use a 5x7 calendar/planner that I bought at Walmart. I

keep all family appts on the calendar but on the daily entries I record

the following things on a daily basis:

Temperature. I have been running a low grade fever for over 3 months

now, Drs aren't concerned, but I take my temp daily when I am

comfortable and nonactive.

Pain level rating at days end. A scale of 1-10 with 10 as the worst I

record my pain for the day. Doing this has allowed me to see that level

slowly going down with my new meds.

Symptoms that were present that day. I have been ill with other

entertaining viruses lately so I am keeping track of normal vs diarhhea

days, good eating days vs zero appetite days, achy knees, knots under

the skin on my hands, this type of thing.

So far keeping a journal of these things has helped me see that I am

gaining on my symptoms and that I am improving. This has also helped

both my GP and RA Drs so see what is happening and when. And when I

have a great or happy event I like to jot that down too. Sometimes

seeing all the pain entries gets a little overwhelming so adding in the

good things helps to neutralize things.

Diane,

You're welcome. Although my explanation was detailed and

complex, that doesn't mean we must comprehend our experience

at that level before we act to protect ourselves. As Doug more

eloquently said (paraphrasing) Go into any midwestern basement

and smell the mycotoxins (or whatever it all is). But don't do that!

We must act without knowing, but that is exactly what gets us into

trouble. So we must do what you are doing and ask questions.

We still don't know - absolutely - but we know more than we did

and hopefully we will make better choices with less harm.

This is why the VCS test is a first screening for toxic exposure. I

remember when I first began getting sick I was teaching and couldn't read my

teacher's manual and had just purchased new glasses about 3 months before. I was

using my glasses on but had to use a magnifier to see the print! Very

scary.

seems like it should be a no brainer, check this out.

You are so welcome. I used to live near Present Moment, I miss being

able to go there whenever the urge hit. Not many places you can go and

find everybody is headed in the same direction, positive personal growth

in every aspect of ones being.

> My blood counts were good today - WBC (3.9) not escalating and I did check

with my Consultant today about being off Glivec. He said until we have the

results I should stay off the med.

> Silly question from a beginner!... can you explain what is tested for in a)

blood (which i know) b)bone marrow and c) cytogenic pcr? I am confused!

__________________________________

Hi Beth,

First thing....to say that your white count has not increased is not really

saying much, here's why. It will be about the last thing to increase. Let's

say you have a bmb.....and the results is 10% ph+ or 2 leuk cells in 20

analyzed.

You want to know if this 10% is increasing....it could easily go back up to 100%

(what you probably were at dx) before your white count would necessarily

increase. This would be an example of a relapse and it would not show in your

white count.

The same thing with your pcr......if that is re-tested and it goes up

significantly, then that could signal a relapse, but the white count may be

unaffected.

---------------------

what is tested from blood:

From peripheral blood, you can do the pcr test and this is the most sensitive

test for residual cml cells but it is more accurate when the % of cml cells is

low.

When you have a bone marrow biopsy....this is the cytogentic test. They actually

try to get 20 (usually) dividing cells and look at the chromosomes and see in

what % of these 20 cells do they see the Philadelphia chromosome (the

translocation that causes cml).

The pcr, usually done with peripheral blood but could be done with bone marrow

aspirate (but should be done with the same material each time for

comparison)....this is a molecular test. It is a hard test to explain (which I

can't) and even understand, but it is determining the % of the bcr-abl that is

found. The result can be in log reductions or %.....or now a international

standardization no.

So, if you doctor is just doing a cbc blood test and looking at the white count,

that does not tell you if things are changing while you are off

drug............or maybe he has done other tests also?

By the time the white count changes above normal, you probably have close to

100% ph+ in the bone marrow.

If the white counts if just varying within the normal range, that does not tell

you much because other things can cause that change. I suppose if it

consistently went up week by week, that could be an indicator....but again not

the first one.

I hope this makes sense.

I kind of cringe when people say 'my white count has not gone up' because that

is not proof that a relapse is not occurring. Eventually, the white count will

go up (just like at dx).

My best to you. I know you will get good care and advice from HH.

Hope everyone is healthy and having a wonderful Christmas day. Thanks to all of

you who have answered my questions these past weeks and to those who have posted

their wonderful stories of encouragement. This is a great site for both

information and inspiration. Marilyn
