Dear Sammy,

I gave this my best shot. I am usually pretty good with law. But I

must say there are a few Federal Law questions that I need my retail

Textperts and Della my resident pharmacist to check out. Some of the

Fed laws are less strict than CA so my state law may supercede them.

Where I would have said all the above your book indicates that there

is only one right answer. So lets hold out on the law until we hear

from , Dora, , Karin and Tracie and Della and perhaps a

few others.

Does your book provide answers?

27. Which drug is used as a " RESCUE " from toxicity of methotrexate?

A: Ipecac

B: Taxol injection

C: leucovorin

D: acetylcysteine

Jeanetta's Answer: C: leucovorin an active form of Vitamin B complex

Folate. MTX depletes the patient of Folic Acid and leucovorin is the

antidote for that drug and any other drug that would adversely

affect the patient in the same way. There are other antidotal

qualities for leucovoring. To read more about what else it does try

A is for poisonings, to cause vomiting, but as of 2003 the new

guidelines suggest just say NO to Ipecac!! Because it can cause

prolonged vomiting and lethargy similar to drowsiness that might be

caused by an overdose of sedative pills, which if mistaken by

emergency personnel could complicate the therapy and undermine the

recovery. It also may not empty the stomach of poison completely and

it could linger inside the GI tract causing a child to vomit up

other antidotes or even to cause acid reflux and burning of the

esophaus.

B is a chemo drug for breast, testicular and ovarian cancers among

others.

D is Mucomyst for Tylenol Poisoning and breathing treatment

Jeanetta's Answer: A Shake Well Suprax is also known as cefixime, a

cephalosporin antibiotic, is available as a powder for oral

suspension, strawberry flavored, when mixed properly it provides

100mg/5ml (or l teaspoonful). After reconstitution the suspension

may be kept EITHER refrigerated or room temperature, up to 14 days.

A: famotidine

Jeanetta's Answer: C: lansoprazole, also known as Prevacid, the

second PPI to go on the market. MOA : binds to the enzyme that would

otherwise initiate the energy to turn on the pump inside the

parietal cell allowing acid to be secreted. By blocking

phosphorlyation the ATP can not be broken down to ADP to turn on the

H+/K+ ATPase Pump System. So basically the cell can make as much

acid as it wants, but it just cannot be secreted! Same family as

Nexium the purple pill.

A and D are H2 Antagonists and work by blocking the H2 from entering

the H2 receptor of the parietal cell of the GI tract. (examples:

Tagamet and Zantac)

B misoprostol, also known as Cytotec, is used to prevent acid caused

by NSAID's when taken properly. It also is a controversial component

of the drugs used for MTP (medical termination of pregnancy). As

such, misoprostol causes uterine contractions in order to expel the

embryo and placental tissue. It is sometimes paired with

mifepristone, which blocks the action of Progesterone, causing the

uterine lining to wear thin and detach from the wall of the uterus

OR with methotrexate, a common Chemo drug, that stops cell division.

Jeanetta's Answer: A: ciprofloxacin, Also known as Cipro, quinolones

are synthetic antibiotics with less resistence associated with them,

but with higher GI associated with them, in general more expensive

than cephalopsorins. Cipro has been one of the drugs in the media

since 9-11 to fight the deadly Anthrax.

56. Which of the following drugs can be administered via aerosol?

A: pentamidine (IV antibiotic for pneumocystis carinii)

B: chloroquine (antimalarial Tx, PO tabs)

C: fluconazole (= Diflucan antifungal usually IV or PO)

D: miconazole

Jeanetta's Answer: D: miconazole, an antifungal that is found in

topicals, including aerosol, for jock itch and anthlete's foot.

74. Consider the following prescription:

Morphine SO4 10mg

Codeine PO4 10mg

Cherry Syrup 5ml

Ethyl Alcohol 5ml

qs water to make tbs

How many 20 mg morphine SO4 tabs would be required to make a pint of

this mixture?

A: 32

B: 16

C: 8

D: 4

Well now this is a nice compounding question:

1 tbls = 15 ml

1 pt can be determined to be two cups, one cup is 8 ounces.

1 ounce = 30 ml

2 Tbl = 30 ml

8oz x 30 ml/oz = 240 ml

2 cups x 240ml/cup = 480 ml

(other sources will say 473 or 500 ml, all three are correct and

commonly used)

Using ration proportion:

10 mg MS……….X mg MS

-------------- = ------------------

15 ml……………..480ml

Cross Multiply = 320 mg MS are needed.

Each tablet is 20 mg answer is 320 mg/ 20 mg tabs = 16 tabs

98. Under Food, Drug and Cosmetic Act, which of the following

information is not required on an outpatient dispensing label?

A: name and address of the pharmacy

B: date the prescription is filled or refilled

C: name of the prescriber

D: telephone number of the pharmacy

Jeanetta's Answer: This is interesting, because I teach that all

items are required on the dispensing label. But if I had to choose

one I would say the telephone number of the pharmacy. I would like

very much for my resident pharmacist Della from Texas to answer this

one. And if my answer is wrong explain why and where can I find this

info?

believe special forms DEA 222 are also required for CI and CII. But

again I would like Della to take this one.

Jeanetta's Answer: WOW! Originally it would have been C, but now I

believe many use it for D. So the `primary' benefit is different

from location to location. But I will go with C.

A: Cevalin (aka Celin, an ascorbic acid supplement (Vit C) )

B: Celexa (SSRI antidepressant)

C: Celestone (betamethasone, a gluccocorticoid, a steroidal anti-

a " generic trade " )

be one answer according to the author of your book. I suppose D. But

again Della??

be placed in the pre-pack log book. Della confirm please?

Hi Sammy,

I found answering all the questions in one post to be confusing.

101. If a medication is noted to be expired, the pharamacy

technician should:

A: discard the medication

B: notify the DEA if it's a controlled substance

C: adhere to the pharmacy's return procedure

D: all the above are correct methods

C: Adhere to the pharmacy's return procedure

All pharmacies should have a policies and procedures manual available for

reference (except maybe the smaller independents.)

C: The patient's age. The patient's age is relevant only to the drug/dose

which the pharmacist will verify later.

Hi Jeanetta-

We used to pentamidine via neb for our PC AIDS patients. We'd

reconstitute a 300mg vial with 6cc swfi via neb over 30-45min. The

patients would come in every 4 weeks for treatment.

There's a commercially available product called Nebupent.
