One must evaluate the changes during the treatment period as well as the changes after the treatment period. Most children benefit from another round of 40 two to three months later. After 80 treatments it becomes very individualized. We recommend that if there has been significant improvement during the first two blocks of therapy, then it might be advantagious to have the child get anywhere between 10 to 20 treatments during physiologic or developmental growt spurts. Now, depending on the age of the child, any given CP child could go well over 200 treatments over the course of several years. Guest guest Posted June 10, 2003 Report Share Posted June 10, 2003 , The challenge with such generalities right now is while many parents (like me) are dogmatic on HBOT as a helpful generic treatment for our kids, very, very few are able to pursue therapies optimally. Mostly it comes down to financial and travel issues in determining the HBOT program for their child. With that said, I believe the more optimal method to pursuing HBOT is to get a SPECT scan, complete 40 sessions, and get a second SPECT scan. Then, wait two months or so and observe your child's changes. why? The SPECT scans will demonstrate the specific metabolic / blood flow changes in the child's brain. The observations during the HBOT, and very importantly two months after the HBOT completion will enable the parent to see some of the improvements that will hopefully occur. The McGill study showed that 82% of its study kids improved during the two months of HBOT only an average of 15 times their base improvement rate (with pt alone). But, what is not determined, is if a child, who doesn't make much improvement during that time, can make improvements with another 40, or 80 sessions. Certainly, many near drowning kids seem to need many more sessions to make good improvements as compared to child with less injuries. In the end, it has to come down to a parental determination. But, I would not wait a year to start the next batch of sessions. I would (optimally) pursue HBOT for 40 sessions, take 2 - 3 months off (rest), pursue 40 more sessions take 2-3 months off then possibly pursue 20 session blocks. As a general note MORE IS NOT BETTER!! Further, note that while the child is having HBOT, it is great for them to get ancillary therapies. Movement therapies, myofacial, cranial sacral, speech, etc. HBOT will make the brain healthier for most kids, but the other issues are how to help that child overcome their bad movement habits, as well as attain higher developmental levels. HBOT can only help, but movement and other therapies are required to be successful. Take care. It would be very unfair to give a " text-book " answer to your question of: " Does one size fit all? " (obviously No). As Ed (and others) have pointed out, if you start and end a treatment session with SPECT scans and other brain imaging this is the best way to really know if a person needs the treatment, a very helpful manner to assist in establishing the proper protocol for that individual patient and to avoid the " guessing-game " . It is also the better way to establish the need for re-starting HBOT after a long break in the routine. Some may need more treatments than others. When I first heard of HDOT I understood immediately why this would help my son and next week we will complete our 700th treatment. This is not a fast process, however it is one that works. My son has autism which means for one reason or another there are no little tiny blood vessels supplying oxygen to his right front temporal lobe. This is where the five senses are controlled by the brain! They call that autism. Now with CP It is the mid brain that doesn't have the necessary supply of oxygen to control physical function. The severity depends on the lack. With mental retardation it is the cortex. Unless you stop seeing improvement after trying " 40 in a row " then I think you are not using your common sense if you quit. Normally, parents do not need a SPECT Scan to tell them their child has improved with HBOT. Again, every case is unique, I certainly would not discourage treatments in the hundreds if there was continued improvememnts. THis is the call of the parents and the supervising physician. It is " nice " for the parents to see the changes to feel encouraged. Particularly for some kids who have a long road ahead of them (like my ). It can give some information if a lot of changes are not seen in the SPECT's. But, even that scenario is less certain. SPECT is actually more important to fight for reimbursement. A number of people have noted that some kids did not show improvements for many, many HBOT's then they take off. So, who really knows? RE: Rose's 700 sessions.... I have to believe angiogenesis is long over after 100, or 200 sessions (depending on the child). This points to other recuperative mechanisms at work. Dr. Harch has informed me that HBOT support axonal sprouting, which is supportive of plasticity growth and hopefully further neuronal development. I believe Dr.Harch (possibly someone else) has mentioned that it appears neuronal stem cells are being developed and supported by HBOT as well. This would better explain continued improvements for large numbers of continued HBOT. Personally, I believe HBOT is supporting all these mechanisms, and more. But, rebuilding a brain, is not like healing a broken bone. So, we really have an incredible amount to learn. And, of course, it comes down to the basic challenge, " how can I get my child treated? Dr. Neubauer has a scanner in his clinic. Dr. Harch has a fabulous scanner in one of his hospitals. We are sending patients to Dr. Amen's clinic in Fairfield (he is very well established).
