I have PBC stage 4 and AIH I don't know what stage and IBS with is a big pain in the you know what. Is the meaning of the meld score the same in all Countries? The technical answer to your question is that the MELD score is used to predict the probability of three-month mortality. It is a statistical model that describes the relationship between an outcome (in this case, 3-month mortality) and other variables (risk factors) theorized to predict the outcome. Like all statistical models, the outcome is predicted with some error/ uncertainty. But the medical literature has shown that the MELD score is a " better " predictor of three-month mortality then other models (such as Child- Pugh Model) . As your MELD score increases towards 40, the statistical probability of death three months hence increases. Most of this increase happens at MELD scores higher than 20. That is, each additional MELD point does not equal the same increase in probability. At a relatively low MELD score like 10, the probability of death is very low. Because the model is simple to use (three lab values), is objective and has been shown to be a good predictor, it is used in the US to allocate livers for transplant. It is not used in Canada for this purpose. So while the statistical meaning of your MELD score is the same in every country, its practical meaning in terms of determining if and when you will get a transplant varies. I should add that I am not aware of any studies that show if the MELD score is a good predictor of your present quality of life or of your risk of developing certain complications. Also, your Doc should know. If they don't, I'd look around for another specialist!
