It just asked me basic questions, no personal info other than State.I dont really see the point of it, or how it will be useful to educate any doctors. It was very vague and generalized. - It asked me my age, sex, race, how long since DX, what kind of doc I see. - It asked me if I had been educated by my doctor - I said no, I have been educated on the internet. - It asked me if I was bothered by the HCV sides - I said some. [my labs are normal] - It asked me what stage I was - I said maybe early or mid. - It asked me if I had had a biopsy - I said no. - It asked me if I was afraid of TX and the sides - I said yes. [who wouldnt be?] - It asked me what I thought my chances were with the old drugs - I said maybe 40%. - It asked me what I thought my chances were with the new drugs - I said maybe 60%+. I am confused on how this is helpful, no box to write something, was too basic and I felt the survey will not help out at all.
