I have a lot of pain in the back area plus when I walk it feels like my buttock/ hips and my upper leg/ thigh get so tired they will just not move. I have to use a cane if I am going to walk a good piece. Good luck on what ever you decide to do. Ann E Lumbar Fusion I had an appointment this last week with a neurosurgeon. After reviewing my MRI, he said that in addition to a herniated disc in L4-5 there was moderately severe spinal stenosis there. There is also a bulging disc at L3-4 and L5-S1. The spinal stenosis is caused by spondylosis indicating instability, broad based disc herniation, and congenitally narrow spinal canal. He said that I might need a fusion at this level. He does not do fusions and is sending me to a doctor in Spokane, WA. My main symptoms are numbness, pins and needles and mild weakness in the legs and feet. He said my muscle strength and reflexes looked good as did the pulse in the feet. I haven't had a lot of pain. He didn't paint a rosy picture about the fusion..said only about 7 out of 10 work. He also said that a fusion could make things unstable above and below it. I am scared to death about having a fusion done. He said it would be between 3 and 6 months recuperation. The numbness in the feet, etc. is driving me crazy and really getting me down emotionally. I am wondering if anyone can identify and if anyone has had a fusion in this area and how it went for you. It really helped with the stenosis, and the tingling, numbness leg problems etc.... (up until this recent accident of course). My father has had lumbar fusions though, with a lot of success. His took, though now some years later he has to have another fusion. Not in the same place, but because over the years just about his whole vertabre disc are degenerating due to DDD. Its very important that you do not try to do to much after so that the fusion will work. Also I have heard that a fusion always takes better with your bones (usually taken out of the hip) then with a cadaver! Also if you are smoker, you should quit because that can have a major impact on the fusion taking as well! Hope any of this helps! Guest guest Posted February 27, 2005 Report Share Posted February 27, 2005 hi sharon, I am 22 months post op on fusion at L/4 L/5 as well as cage insertion to replace a disc and i have had no relief from the sympyoms that i had before surgery, plus i have a lot of added problems. But like i have said to everyone else it is your choice and your choice alone to make. I opted for the ops because i have 3 young children and thought any chance to function normally was better than anything. They never said anything to me about instability in my spine, but then I didnt have a fusion either, so their may be a difference there. Glad you are not a smoker. I am unfortunately, and have tried to quit many times. That is another reason I have not scheduled the fusion in my neck yet, because I want to quit before hand, to up the success of the fusion. It did not relieve the numbness and tingling that I have in my feet because my nerves were compressed too long. It did relief the lack of proprioception (being able to tell where your feet are) that I had. What your doctor is talking about is adjacent segment disease. Because a level is fused it does not move like it used to. This places stress of the level above the fusion as well as the one below the fusion. In my case, it has always been the level above the fusion. Because of the fusion, I developed a degenerative scoliosis at L3-4 and on x-rays the spinous processes are so close they appear to be "Kissing". I have developed adjacent segment disease in my neck from being fused from C4-7. Now I am going to have to have C3-4 fused. By the time that I had the L4-S1 fusion, I had no Achilles reflex. It is the spinal stenosis that would have me worried. The stenosis is the thing that is placing pressure on the nerves. My diagnosis, when I finally found a doctor competent enough to make one, was spinal stenosis secondary to vertebral instability. Is there any possibility that you could qualify for an artificial disc?? They are no longer in clinical trials. They have been approved by the FDA for use in the lumbar spine. If this could meet your need, then, you would not need to worry about adjacent segment disease. Right now they are only using them for one level, but if the L4-5 level is the only one being proposed in the fusion, this may be the answer to your prayers. Then, you would also not have to worry about autograft ( your own bone harvested from your iliac crest) or allograft (donor bone, usually cadaver). If you have any questions, I will try to help. The better ones should be approved this summer, check the links sections for info on them. As for the stenosis its a pain I know I have it in my neck and back. They did fix it somewhat in my neck with surgery but I had to have it fused there too. The nerve pain in my neck was off the charts though I had no choice but to do it then. It has helped alot but I still have to be very carefull lifting or anything that pulls on my neck will cause it to hurt for several days. Its been a year since my surgery. As for the numbness in your feet that drove me crazy before too, now I wish it was numb, cause now the nerve hurts all around my foot and half the time I can not wear shoes at all.
