Guest guest Posted August 8, 2006 Report Share Posted August 8, 2006 Hi all, I fill out a form when I get my shots that asks different questions but at the bottom it states " It is not effective in all people " I am better on the Xolair but I am still couphing even right after I get my shots. It has only been four days today. I use Flovent 220 and Singular and Neb with Xopenex. I am on the 375mg of Xolair and my IGE is through the roof at 1273. I sent a link before that talks about using both the Xolair and Immunotherapy. I am going to the doc next week to talk about doing this. He has mentioned it before but I was confused about how that would work like they might counteract each other. Link http://www.ccjm.org/PDFFILES/Rambasek304.pdf It says: Omalizumab is most effective in people with low body weight, lower IGE levels, and frequent hospitalizations in spite of multidrug therapy. It also states that the IgE level should not exceed 700 IU/ml because the volume of the injection would likely be too large. So... this to me means with my weight of 196 and my IgE of 1273 So does this mean I shouldn't be taking Omalizumb? Wow! It does help though. I stopped for a couple of months and was worse. I still need help so I am looking toward the Allergy Shots too. Next I read this below so I am confused. Omalizumab has not been well studied at doses above 750 mg per month and is not FDA-approved above this dose. This is primarily because using doses greater than 750 mg per month would be cost-prohibitive (see above discussion) and would require more than two injections per month because technical issues limit how much this drug can be concentrated. I couldn't find the place where I read about combining Allergy Shots with Xolair. It said something to the effect that the IgE that does not get bound by the Xolair is treated with the Allergy Shots. Like when all of the Xolair is used up there is still IgE to deal with. I will continue trying to find this info. Some patients with asthma or allergic rhinitis currently receive allergen vaccines ( " allergy shots " ). Occasionally, patients receiving immunotherapy exhibit a tendency for serious IgE-mediated (anaphylactic) reactions. Such patients may be unable to advance the dose of immunotherapy injections to therapeutic levels. Omalizumab, by decreasing the severity or frequency of these reactions, may enable these patients to achieve therapeutic ( " maintenance " ) doses of immunotherapy. " I did of the search of the document (CTRL+F) and didn't see anything else regarding immunotherapy. Do you think it was from another article? Guest guest Posted August 9, 2006 Report Share Posted August 9, 2006 Hi , If it makes you feel better, I am far from cured despite the Xolair. I have not had the miraculous, life-altering effect that some have described. I am better (less pred, fewer hosp) but I am still ++ symptomatic. In fact, I am on a sick day from work today (I am a nurse) since I am too sob to walk around much. As for allergy shots, I am not sure about combining them with xolair but I do know the evidence that they work for severe asthma is poor. In fact, my pulmo was so not convinced, he put it up to me to decide to try it or not. I didn't. I do have lower IgE than you but I was measured while on pred so that can mask the results. The only advice I have is that if there is no exhorbitant cost for you (co-pays etc...) and since Xolair has few if any side effects, I would stick with it for a couple of years to see.... ps the way I figure it, a little bit better is still an improvement...
