Seeking Patients with Asthma and Allergies for Survey Research Project

Ovation Research Group, in conjunction with a leading pharmaceutical

company, is interested in gathering information about people's

experiences with prescription treatments for asthma symptoms triggered

by allergies. We are inviting eligible people to complete an online

questionnaire, followed by a 30 minute follow-up telephone call. The

questionnaire will take approximately 15-20 minutes to complete. The

questionnaire will ask you questions about how your allergic asthma

symptoms may affect your ability to perform regular activities, such as

sleeping, working or going to school, and participating in leisure

activities. During the follow-up call, a study staff member will review

the questionnaire with you and ask you some additional questions.

To be eligible, you must:

* Have asthma and allergies

* Currently taking Xolair® or Advair®

* Started Xolair® treatment within the last 4 to 12 months OR have

been taking Advair® for at least 12 months

* Between 18 and 55 years of age

* A non-smoker for at least 6 months

* Have internet and telephone access

* Be willing to provide your feedback over the telephone.

Eligible and interested participants will be paid $75.00 for their time

upon completion of a telephone interview.
