Xylitol is not alcohol based it is a sugar alcohol which is a little different than an alcohol that is made through fermenting. Sugar alcohols occur naturally in foods. Xylitol is a natural substance found in fibrous vegetables and fruits as well as corn cobs and brich trees. Vanilla extract is made by soaking the beans in ethyl alcohol & fermenting process and makes me think that would be more harmful, but I am not an expert. The hydrogenation of Xylitol is a partial hydrolization of the starch. The reason it helps fight tooth decay is because it cannot feed bacteria or yeast. Using xylitol helps to raise plaque pH, thereby reducing the time that teeth are exposed to damaging acids, as well as starving harmful bacteria of their food source. I have liquid Stevia which I guess is processed and therefore not good. But I've only used it a few times to sweeten my lemon drink occasionally. Normally I drink it without a sweetener.
