When you had your biopsy did the doctors say they could tell from the biopsy that you have AIH? My GI doctor said no. But in my reading there is a different pattern of inlammation. Also, I know some of us have diagnosis of cirrhosis. If your biopsy says cirrhosis wouldn't it be more difficult to determine AIH? I see Dr. Czaja in August and am just curious about others biopsies. I just don't understand how I could develop cirrhosis and have moderate active inflammation when I've never drank or did any drugs. But with a biopsy that says cirrhosis part of me feels like I need to go to Mayo and then another part of me thinks not. I feel OK for now. Mainly a lot of fatigue,depression etc-but nothing that feels life threatening. I guess I'm in a little bit of denial!? I have had a lot of health problems the last few years so maybe the appointment won't be for nothing. I just feel like I'm wasting their time. Guest guest Posted July 9, 1999 Report Share Posted July 9, 1999 Hi Susie, The biopsy is one of several pieces necessary to diagnose AIH. Unfortunately, there is no definitive test. Diagnosis is made by considering a variety of factors, including blood tests and symptoms. Diagnosing autoimmune diseases is as much art as science. That is because most autoimmune diseases present in very different ways for each patient. Not everyone has the same symptoms, antibodies, or biopsy results. For example, my biopsy looks more like damage from a medication reaction than from AIH, but other factors are more indicative of AIH. Positive ANA's are nearly always seen in autoimmune disease. However ANA can become positive from aging, viral infections, and some prescription drugs. However, levels greater than 1:1280 almost always indicate an autoimmune disorder. HTH. Guest guest Posted July 9, 1999 Report Share Posted July 9, 1999 Susie, a person will develop cirrhosis when the liver becomes so damaged that it can no longer repair itself. Then the liver becomes scarred and will begin to shrink and will eventually shut down in the late stages of cirrhosis. That is a very common conception that has been around a long time, that you only develop cirrhosis from too much alcohol or drugs or both. Well in Autoimmune hepatitis and primary bilarary cirrhosis and primarary sclerosing colangitis, it is a person's own immune system, that for some unknown reason, decides to attack the liver or the liver bile ducts or both, causing the cirrhosis. I know when I was diagnosed with AIH with cirrhosos, it was like, " HELLO, HOW DID I GET CIRRHOSIS WHEN I HARDLY DRINK AND DON " T DO DRUGS? " It is a heard pill to swallow when you get such a diagnosis. I was in denial for a while. But I overcame the shock and moved on. Hope this helps!!! Lori C. My most recent tests, however, are negative for both. Talk about confusing! As far as what a doc will give the most weight to, that is where the art comes in. Some are strictly numbers, others are mostly symptoms. I prefer someone who looks at the total picture. It is confusing. But then, if it wasn't, why would we need to big buck docs?? I also tested positive for the smooth muscle antibody. That together with my symptoms that I had finally led me to my diagnosis. I went undiagnosed for years by my local Drs. It was finally an oncologist (cancer DR.) that saw me and read my chart and looked at me and said I had AIH w/ cirrhosis. Then he sent me to Dr. Czaja & Dr. Marotta, the GI Drs. at Mayo in Rochester, Mn. Hope this helps. Lori C. Guest guest Posted July 10, 1999 Report Share Posted July 10, 1999 Susie, I would say go to Mayo for the 2nd opinion. If you have any doubts, that would be your best bet. I know I did not feel that it was life threatening for me either, I was over weight and had been all my life, and just figured it was blood pressure or my heart, then I finally went to my local doc and all my blood levels were so low, they sent me to Mayo for bone marrow biopsy to see if I had Leukemia, and then it took Mayo about 3 1/2 weeks to figure out that it was AIH. Even though my liver enzymes had been elevated for years. At least since 1989, when I was 22. It's real hard to accept that you may have a chronic disease, but once you get to that point of acceptance, it becomes so much easier. I'm not gonna preach God to anyone, but I have to say that the only way that I came to acceptance of my disease and made it through my transplant process & am still going on today is through my faith. My pastor & my church were so very good to me throughout the whole thing. God has truly been good to me. How many people can say that after going through such a traumatic experience? I really would not change anything, I have learned so much and met so many wonderful people along this journey. In a real way it has almost been a blessing. Go figure!! I was sent over to him to confirme my AIH diagnosis which was made by an oncologist there from Mayo, Dr. Letendre. Guest guest Posted July 15, 1999 Report Share Posted July 15, 1999 Dear Jody: Although I don't know what makes a doctor choose one method or the other, my hepatologist just told me the other day that a biopsy done laparoscopally (sp) was the best route because that way they can see the damage to liver, whereas when they do a needle biopsy, they are just looking at one tiny section and it might not be indicative of the entire 'liver picture.' When you go back home it still hurts in some positions but if you change it the pain go away. You only have to look for the best way for you to lie down. It's so difficult to fins the words in English ! I hope you got it. ) Don't worry abour that. That was to make sure it didn't start bleeding. It gave me a big headache, for which they would not give me anything for, because the doctor wasn't there, and no one had any painreliever. In a hospital no less! Probably would have cost 20.00 a pill anyway! Guest guest Posted July 20, 1999 Report Share Posted July 20, 1999 I wonder if prothrombin time at the time of biopsy has something to do with how long you have to lie down after a biopsy? I had my liver biopsy in the hospital so no problem about lying down - that's all I COULD do. I know that they considered me high risk for bleeding and told my husband that they had to collect 5 units of plasma before they did the biopsy. I've been told by my doctors since then that a liver biopsy is a " risk " procedure. I've also had a lung biopsy, just last year. They wanted to keep me in the hospital overnight but I convinced them that I'd go to the hotel and stay in bed so they let me go after a couple of hours bed rest after the biopsy. I'd be a little nervous about getting up and being active soon after a liver biopsy - at least ask your doctor about the bleeding possibilities after a laparoscopy.
