We are trying to spread the word about Lyme disease and its associated

complications to the field of speech-language pathology, a profession

uniquely qualified to diagnose and treat some difficulties which those

with this condition have experienced. To help us out, please

participate by taking our brief survey, which can be accessed by

clicking any of the links provided in this message. With the survey

only being available for 1 more week, we hope a large number of you who

haven't already done so choose to participate, as thus far the

response rate has been very low and our results reflect the impressions

of only a limited number of people. The time to participate is running

short and the survey will cease being available March 12. (Please be

assured the information you provide will be kept confidential and can in

no way be used to individually identify you. Completing this survey will

not generate unwanted email solicitations from the researchers or any

third-parties.)

Survey completion is quick and easy, though individuals taking it should

be aware of the following instructions:

1. The survey must be completed in a " single sitting " as there

is no provision for saving responses and returning to finish at a later

time. There is no time limit once you begin taking the survey so feel

free to take as long as you need.

2. Navigate through the survey by clicking the " Next " button at

the bottom of each page. Only click the button after the page has been

completed, as you will be unable to go back to a page after you have

left it.

3. When you arrive at the final page click the " Done " button and

close the window the survey opened in.

4. Optimize viewing by maximizing window size if the survey does not

open as a full screen. Minor compatibility problems with some internet

browsers may cause the survey format and spacing to be disrupted on your

computer. This will in no way affect you ability to take the survey or

compromise your answers.
